基于“3H”理论的全程优质护理模式对初产妇妊娠结局、泌乳功能及自我效能的影响

Effect of Whole Course Quality Nursing Model Based on “3H” Theory on Pregnancy Outcome, Lactation Function and Self-efficacy of Parturients

目的:探讨基于“3H”理论的全程优质护理模式对初产妇妊娠结局、泌乳功能及自我效能的影响。方法:选取2017年1月-2018年5月笔者所在医院收治的初产妇148例,随机分为优质组(74例,给予基于“3H”理论的全程优质护理模式)与对照组(74例,给予常规护理)。记录并比较两组护理后妊娠结局、产后泌乳功能及自我效能评分。结果:优质组剖宫产率低于对照组,早产、产后出血、胎儿窘迫及新生儿窒息发生率均低于对照组,新生儿Apgar评分高于对照组,差异均有统计学意义(P<0.05)。优质组产后泌乳始动时间早于对照组,差异有统计学意义(P<0.05);两组产后泌乳量评分均呈上升趋势,且优质组评分均高于对照组,差异均有统计学意义(P<0.05)。结论:采用基于“3H”理论的全程优质护理模式可明显改善初产妇妊娠结局,缩短产后泌乳始动时间与促进产后泌乳功能,增强自我效能,值得推广应用。

Objective:To discuss the effect of whole course high quality nursing model on pregnancy outcome,lactation function and self-efficacy of primiparas based on“3H”theory.Method:A total of 148 parturients admitted to the hospital from January 2017 to May 2018 were selected,and were randomly divided into the high quality group(74 cases,given whole course high quality nursing model based on“3H”theory)and the control group(74 cases,given routine care).The pregnancy outcome,postpartum lactation function and self-efficacy score were recorded and compared between the two groups.Result:The rate of cesarean section in the high quality group was lower than that in the control group,and the incidence of premature delivery,postpartum hemorrhage,fetal distress and neonatal asphyxia in the high quality group were lower than those in the control group,and the neonatal Apgar score was significantly higher than that in the control group,the differences were statistically significant(P<0.05).The onset time of postpartum lactation in the high quality group was earlier than that in the control group(P<0.05).And the score of postpartum lactation in the two groups showed an upward trend,and the score in the high quality group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:The whole course high quality nursing model based on“3H”theory can obviously improve the pregnancy outcome of the primipara,short postpartum lactation start time and promote postpartum lactation function,enhance self-efficacy,which is worthy of popularization and application.
