经输尿管镜钬激光术对尿道狭窄合并尿道结石的治疗价值及安全性

The Value and Safety of Transureteroscopic Holmium Laser in the Treatment of Urethral Stricture Complicated with Urethral Calculi

目的:探究对尿道狭窄合并尿道结石患者实施经输尿管镜钬激光术的治疗效果及安全性。方法:选取2017年6月-2018年6月医院收治的80例尿道狭窄合并尿道结石患者开展对照研究,按照随机数字表法分成两组,对照组40例实施常规手术,试验组40例实施经输尿管镜钬激光术,比较两组手术疗效。结果:试验组总有效率为92.50%,明显高于对照组的75.00%(P<0.05)。试验组总出血量及膀胱痉挛次数均明显少于对照组,手术用时短于对照组(P<0.05)。治疗前两组生活质量评分比较,差异无统计学意义(P>0.05);治疗后试验组生活质量评分明显高于对照组(P<0.05)。试验组并发症发生率为5.00%,明显低于对照组的15.00%(P<0.05)。结论:对尿道狭窄合并尿道结石患者实施经输尿管镜钬激光术的效果确切,在减少尿道黏膜损伤的同时可提高碎石率,减少并发症,手术治疗安全性显著高于常规手术,值得推广。

Objective:To investigate the efficacy and safety of transureteroscopic holmium laser in the treatment of urethral stricture complicated with urethral calculi.Method:From June 2017 to June 2018,a total of 80 patients with urethral stricture complicated with urethral calculi treated in the hospital were collected for a controlled study.According to the random number table method,patients were divided into two groups.And 40 cases in the control group was treated with routine surgery and 40 cases in the experimental group was treated with transureteroscopic holmium laser.Surgical effect of the two groups were compared.Result:The total effective rate of the experimental group was 92.50%,which was significantly higher than 75.00%of the control group(P<0.05).The amount of total bleeding and the times of bladder spasm in the experimental group were significantly less than those of the control group,and the time of surgery was significantly shorter than that of the control group(P<0.05).Before treatment,the score of quality of life was compared between the two group,and the difference was not statistically significant(P>0.05).But after treatment,the score of quality of life in the experimental group was significantly higher than that of the control group(P<0.05).The incidence of complication in the experimental group was 5.00%,which was significantly lower than 15.00%of the control group(P<0.05).Conclusion:Transureteroscopic holmium laser is effective in the treatment of urethral stricture complicated with urethral calculi.It can not only reduce the urethral mucosa injury,but also increase the lithoclast rate and reduce the complications.The safety of the surgical treatment is significantly higher than that of the routine surgery,which is worth popularizing.
