经肛门置管引流治疗直肠癌保肛术后吻合口漏的效果

Effect of transanal catheter drainage in the treatment ofanastomotic leakage after anal preservation operation for rectal cancer

目的探讨经肛门置管引流治疗直肠癌保肛术后吻合口漏的效果。方法选择本院收诊的吻合口漏患者42例,采用完全随机法将其分为观察组(21例)与对照组(21例)。对照组患者采用腹腔引流;观察组患者采用经肛引流。比较两组患者的治疗效果。结果观察组患者的住院费用、吻合口漏后二次手术发生率低于对照组,吻合口漏后住院时间、引流管放置时间短于对照组(P<0.05)。治疗后,两组患者的CRP水平低于治疗前,PA、ALB水平高于治疗前,且观察组优于对照组(P<0.05)。结论经肛引流方法可有效提高直肠癌保肛术后吻合口漏患者的临床疗效及预后质量,值得临床推广应用。

Objective To investigate the effect of transanal catheter drainage in the treatment of anastomotic leakage after anal preservation operation for rectal cancer. Methods Forty-two patients with anastomotic leakage admitted in our hospital were selected and divided into observation group(21 cases) and control group(21 cases) by complete random method. The patients in the control group received abdominal drainage and the patients in the observation group received transanal drainage.The treatment effects were compared between the two groups. Results The cost of hospitalization and the incidence of secondary operation after anastomotic leakage in the observation group were lower than those in the control group, and the time of hospitalization after anastomotic leakage and the time of drainage tube placement were shorter than those in the control group(P<0.05). After treatment, the CRP levels of the two groups were lower than those before treatment, the PA and ALB levels were higher than those before treatment, and those in the observation group were better than the control group(P<0.05). Conclusion Transanal drainage can effectively improve the clinical efficacy and prognosis quality of patients with anastomotic leakage after anal preservation operation for rectal cancer, which is worthy of clinical popularization and application.
