益气活血汤加减联合二甲双胍治疗多囊卵巢综合征的临床效果

Clinical effect of modified Yiqi Huoxue decoction combined with metformin in the treatment of polycystic ovary syndrome

目的探讨益气活血汤加减联合二甲双胍治疗多囊卵巢综合征(PCOS)的临床效果。方法选取140例PCOS患者作为研究对象,按数字表法将其分为对照组和观察组,各70例。对照组接受二甲双胍药物治疗,观察组在对照组基础上增加益气活血汤加减治疗。比较两组的临床疗效、排卵率和妊娠率;比较两组治疗前、后的性激素水平、分泌代谢指标水平及胰岛素抵抗指数。结果观察组的治疗总有效率为94.29%,明显高于对照组的78.57%,差异有统计学意义(P<0.05)。治疗后,两组LH、PRL及T水平均降低,E2水平均升高,且观察组优于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的TC、TG、FPG水平及HOMA-IR均低于对照组及治疗前,差异具有统计学意义(P<0.05);但对照组治疗前、后,TC、TG、FPG水平及HOMA-IR比较,差异无统计学意义(P>0.05)。治疗后随访半年,观察组的排卵率和妊娠率分别为84.29%和45.71%,均高于对照组的64.29%和22.86%,差异具有统计学意义(P<0.05)。结论益气活血汤加减联合二甲双胍治疗PCOS可有效调节患者的激素水平,改善糖代谢、脂代谢及胰岛素抵抗,提高排卵率及妊娠率。

Objective To investigate the clinical effect of modified Yiqi Huoxue decoction combined with metformin in the treatment of polycystic ovary syndrome(PCOS). Methods A total of 140 patients with PCOS were selected as the research objects and divided into control group and observation group according to digital table method, with 70 cases in each group. The control group was treated with metformin, while the observation group was treated with modified Yiqi Huoxue decoction on the basis of the control group. The clinical efficacy, ovulation rate and pregnancy rate of the two groups were compared. The levels of sex hormones, secretion and metabolism indexes and insulin resistance index before and after treatment were compared between the two groups. Results The total effective rate of treatment of the observation group was 94.29%, which was significantly higher than 78.57% of the control group, the difference was statistically significant(P<0.05). After treatment, the levels of LH, PRL and T in the two groups decreased, while the levels of E2 in the two groups increased, and those in the observation group were better than the control group, the differences were statistically significant(P<0.05). After treatment, the levels of TC, TG, FPG and HOMA-IR in the observation group were lower than those in the control group and before treatment, the differences were statistically significant(P<0.05);however,there were no significant differences in the levels of TC, TG, FPG and HOMA-IR in the control group before and after treatment(P>0.05). Six months of follow-up after treatment, the ovulation rate and pregnancy rate in the observation group were 84.29% and 45.71% respectively, which were higher than 64.29% and 22.86% in the control group, the differences were statistically significant(P <0.05). Conclusion Modified Yiqi Huoxue decoction combined with metformin in the treatment of PCOS can effectively regulate hormone levels, improve glucose metabolism, lipid metabolism and insulin resistance, and enhance ovulation rate and pregnancy rate of patients.
