筛骨垂直板行鼻整形术的短期效果观察

Short-term therapeutic effect evaluation of comprehensive rhinoplasty with the assistance of perpendicular plate of ethmoid

目的评价并比较筛骨垂直板辅助鼻中隔软骨、耳软骨行鼻整形术的短期效果。方法回顾性分析自2017年1月至2019年9月就诊的18例女性患者资料。根据是否联合使用筛骨垂直板,将患者分为两组分别进行疗效分析,每组9例。患者采用自体鼻中隔软骨、耳软骨联合或不联合筛骨垂直板构建鼻尖结构,膨体聚四氟乙烯或硅胶假体填充鼻背。手术前后利用三维照相机VECTRA XT拍摄患者面部照片,测量鼻小柱-小叶长度比、鼻额角、鼻小柱-上唇角、鼻尖突出度,并进行统计学分析。结果术后随访1~18个月。联合筛骨垂直板组术前与术后7 d比较,鼻小柱-小叶长度比、鼻小柱-上唇角、鼻尖突出度的差异有统计学意义(P<0.05),鼻额角差异无统计学意义(P>0.05);未联合筛骨垂直板组术前与术后7 d比较,鼻小柱-小叶长度比、鼻小柱-上唇角的差异有统计学意义(P<0.05),鼻额角和鼻尖突出度差异无统计学意义(P>0.05)。结论应用筛骨垂直板组与未用筛骨垂直板组患者术后短期鼻部美学效果均显著改善,两组测量指标改变趋势在统计学上表现出一致性。

Objective To evaluate the short-term therapeutic effect of perpendicular plate of ethmoid assisted nasal septal cartilage and auricular cartilage for rhinoplasty. Methods The clinical data of 18 female patients underwent rhinoplasty from January 2017 to September 2019 were retrospective analyzed. All patients were divided into 2 groups for therapeutic effect analysis according to whether the perpendicular plate of ethmoid was used. Autologous nasal septal cartilage, auricular cartilage with or without perpendicular plate of ethmoid were used to build the nasal tip. The e PTFE or silica gel prosthesis was used to fill the nasal dorsum. The photographs of face of patients were taken by three-dimensional camera VECTRA XT before and after operation. The nasal columella-lobule ratio, nasofrontal angle, nasal columella-upper lip angle and nasal tip projection rate was measured and statistically analyzed. Results All patients were followed up from 1 to 18 months. On the seventh day after operation, the nasal columella-lobule ratio, nasal columella-upper lip angle and nasal tip projection rate of the group of comprehensive rhinoplasty with the assistance of perpendicular plate of ethmoid showed significant differences(P<0.05). In the group of comprehensive rhinoplasty without the assistance of perpendicular plate of ethmoid, the nasal columella-lobule ratio and nasal columella-upper lip angle showed significant differences(P<0.05). Conclusion The short-term nasal aesthetic therapeutic effect after operation of both groups was significantly improved. The change trend of measurement indexes in both groups was statistically consistent.
