加味四黄散外敷辅助治疗下肢丹毒护理研究

Nursing Study on External Application of Modified Sihuang Powder in Adjuvant Treatment for Erysipelas of the Lower Limbs

目的:观察加味四黄散外敷辅助治疗下肢丹毒的疗效及护理体会。方法:将92符合要求的患者随机分为对照组45例和观察组47例。2组均采用对症及抗生素治疗,对照组采用50%硫酸镁湿敷。观察组采用加味四黄散外敷。进行治疗前后局部症状、体征积分评价,记录发热、疼痛和肿胀消失时间;记录(白细胞总数及中性粒细胞计数)恢复正常时间。结果:经秩和检验,观察组临床疗效优于对照组,差异有统计学意义(Z=1.175,P<0.05)。观察组发热、疼痛、肿胀消失时间和外周血恢复正常时间均短于对照组,组间比较,差异有统计学意义(P<0.01)。治疗后,2组疼痛程度、肿胀程度、皮肤温度、皮肤颜色、功能障碍评分均较治疗前下降(P<0.01)。治疗后观察组疼痛、肿胀和功能障碍评分均低于对照组,组间比较,差异有统计学意义(P<0.01)。结论:加味四黄散外敷辅助治疗丹毒能迅速缓解症状,缩短病程,减轻痛苦,促进恢复,提高了临床疗效。

Objective:To observe the curative effect and nursing experience of external application of modified Sihuang powder in adjuvant treatment for erysipelas of the lower limbs.Methods:Divided 92 cases of patients accorded with requirement into the control group with 45 cases and the observation group with 47 cases randomly.Both groups adopted the symptomatic treatment and were treated with antibiotics and the control group was treated with wet packing of 50% magnesium sulfate.The observation group adopted the external application of modified Sihuang powder.Evaluated the scores of local symptoms and signs before and after treatment and recorded the disappearance time of fever,pain and swelling;recorded the time of recovery(leukocyte count and neutrophil count).Results:Rank sum test showed that the clinical effect in the observation group was better than that in the control group,difference being significant(Z=1.175,P<0.05).The disappearance time of fever,pain and swelling and the time of recovery of peripheral blood in the observation group were shorter than those in the control group and there was significant difference being found in the comparison between the two groups(P<0.01).After treatment,the scores of the pain degree,swelling degree,skin temperature,skin color and dysfunction in the two groups were all decreased when compared with those before treatment(P<0.01).After treatment,the scores of pain,swelling and dysfunction in the observation group were all lower than those in the control group and there was significant difference being found in the comparison between the two groups(P<0.01).Conclusion:The external application of modified Sihuang powder in adjuvant treatment for erysipelas can rapidly relieve the symptoms,shorten the course and reduce the pain,which promotes the recovery of the disease and enhances the clinical effect.
