探讨机械通气患者不同时间气管支气管细胞形态学的改变及对患者血气变化、炎性因子、X片的影响

Explore the Morphological Changes of Ttracheobronchial Cells in Patients with Mechanical Ventilation at Different Times and the Effects on the Changes of Blood Gas, Inflammatory Factors and X-ray Film

目的探讨分析机械通气患者不同时间气管支气管细胞形态学的改变及对患者血气变化、炎性因子、X片的影响。方法抽取100例2012年1月至2014年3月进行机械通气的患者,将其分为A组(粘膜正常组)、B组(粘膜轻度损伤组)、C组(粘膜中度损伤者)、D组(粘膜重度损伤者)。分析粘膜损伤程度与呼吸机相关性肺炎的相关性以及机械通气参数与呼吸机相关性肺炎的关系。结果所有患者均实行24 h、48 h、72 h、96 h、1周的机械通气,血气指标的改善情况不同,炎性因子水平变化有明显差别,X片检查出的肺不张、气胸等症状。结论气管粘膜损伤越重,血气指标越差,炎性反应越重,发生肺部合并症越多。

Objective To explore and analyze the morphological changes of tracheobronchial cells in patients with mechanical ventilation at different times and the effects on the changes of blood gas,inflammatory factors and X-ray.Methods One hundred patients with mechanical ventilation from January 2012 to March 2014 were divided into group A(normal mucosa group),group B(mild mucosa injury group),group C(moderate mucosa injury group)and group D(severe mucosa injury group).To analyze the relationship between the degree of mucosal injury and ventilator-associated pneumonia,and the relationship between the parameters of mechanical ventilation and ventilatorassociated pneumonia.Results All patients were given mechanical ventilation at 24 h,48 h,72 h,96 h and 1 week.The improvement of blood gas index was different,the change of inflammatory factor level was obviously different,and the symptoms of atelectasis and pneumothorax were detected by X-ray.Conclusion The more severe the injury of tracheal mucosa,the worse the blood gas index,the more severe the inflammatory reaction and the more pulmonary complications.
