结直肠癌患者血清miR-542-3p和miR-203水平及临床意义

Expression and clinical significance of serum miR-542-3p and miR-203 in patients with colorectal cancer

目的探讨结直肠癌患者血清miR-542-3p和miR-203水平检测及临床意义。方法选取2014年8月至2018年9月收治的结直肠癌患者134例为观察组,于根治性术前及术后3~4周采集全血;同期健康体检者118例为对照组,体检时采血;测定血清miR-542-3p、miR-203及血清癌胚抗原(CEA)表达水平。结果术前,观察组miR-542-3p、miR-203水平低于对照组,CEA水平高于对照组(P<0.05)。术后,观察组miR-542-3p、miR-203水平高于术前,CEA水平低于术前(P<0.05)。而病理学分级越高、TNM分期越高、有淋巴血管间隙浸、有淋巴结转移、有复发患者血清miR-542-3p、miR-203表达水平越低(P<0.05)。CEA与miR-542-3p、miR-203呈负相关(r=-0.569、-0.613,P<0.05)。CEA、miR-542-3p、miR-203ROC曲线下面积(AUC)分别为0.753(95%CI 0.602~0.798),0.680(95%CI 0.523~0.745),0.658(95%CI 0.569~0.779),miR-542-3p、miR-203的AUC相近,皆低于CEA,miR-542-3p、miR-203诊断结直肠癌的灵敏度、特异度差异无统计学意义(P>0.05),皆低于CEA(P<0.05)。结论miR-542-3p、miR-203在结直肠癌患者血清的表达量降低,miR-542-3p、miR-203在结直肠癌的发生发展过程中起抑制作用;miR-542-3p、miR-203具有较高的灵敏度、特异度,可作为诊断结直肠癌的生物学标志物。

Objective To investigate the expression and clinical significance of serum miR-542-3p and miR-203 in patients with colorectal cancer.Methods A total of 134 patients with colorectal cancer were enrolled as observation group,and 118 henlthy subjects who underwent physical examination were enrolled as control groupThe serum levels of miR-542-3p,miR-203 and CEA were detected and compared between the two groups.Results Before operation the levels of miR-542-3p and miR-203 in observation group were significantly lower than those in control group,however,the levels of CEA were significantly higher than those in control group(P<0.05).After operation,the levels of miR-542-3p and miR-203 in observation group were significantly higher than those before operation,but the levels of CEA were significantly lower than those before operation(P<0.05).The serum levels of miR-542-3p and miR-203 were similar in the patients with different age,the maximum diameter of the tumor and the number of tumor(P>0.05).The serum levels of miR-542-3p and miR-203 were lower in patients with the higher pathological grade,the higher TNM staging,the lymphatic vessel space immersion,the lymph node metastasis and the recurrence(P<0.05).Moreover CEA was negatively correlated with miR-542-3p and miR-203(P<0.05).The area under the curve(AUC)of CEA,miR-542-3p,and miR-203ROC was 0.753(95%CI 0.602~0.798),0.680(95%CI 0.523~0.745)and 0.658(95%CI 0.569~0.779),respectively.The AUCs of miR-542-3p and miR-203 were similar,which were lower than those of CEA.The sensitivity and specificity of miR-542-3p and miR-203 in diagnosis of colorectal cancer were similar(P>0.05),which were significantly lower than those of CEA(P<0.05).Conclusion The expression levels of miR-542-3p and miR-203 are decreased in patients with colorectal cancer,which play an inhibitory effect in the pathogenesis and development of colorectal cancer.Moreover MiR-542-3p and miR-203 have higher sensitivity and specificity in diagnosis of colorectal cancer.
