寻常型银屑病患者中Th17细胞、CD4^+、CD25^+调节性T细胞表达水平及意义 被引量:13

Expression and clinical significance of Th17 cells,CD4^+ and CD25^+ regulatory T cells in patients with psoriasis vulgaris

摘要 目的分析寻常型银屑病(psoriasis vulgaris,PV)患者中Th17细胞、CD4^+、CD25^+调节性T细胞表达水平及临床意义。方法选取2017年6月至2018年6月收治的64例PV患者为PV组,另选取同期健康体检者40例为健康对照组,采用流氏细胞仪直接免疫荧光法对外周血中Th17细胞、Foxp3^+Treg细胞及Treg细胞的表达水平进行检测。结果健康对照组CD4^+CD25^+Foxp3^+T细胞及CD4^+CD25^+T细胞水平明显高于PV进行期患者,2组差异有统计学意义(P<0.05);健康对照组CD4+CD25+Foxp3+T细胞及CD4+CD25+T细胞水平明显高于PV静止期患者,2组比较差异有统计学意义(P<0.05)。健康对照组CD4^+CD25^+T细胞IL-10和CD4^+CD25^+TGF-β的细胞水平明显高于PV进行期患者,2组比较差异有统计学意义(P<0.05);健康对照组CD4+CD25+T细胞IL-10和CD4+CD25+TGF-β的细胞水平明显高于PV静止期患者,2组比较差异有统计学意义(P<0.05)。健康对照组Th17细胞水平明显低于PV组(P<0.05)。Pearson相关性分析显示,PASI与CD25^+TGF-β^+T细胞无相关性(P>0.05);PASI与CD4^+CD25^+Foxp3^+T细胞和CD25^+IL-10^+T细胞呈负相关(P<0.05);Foxp3^+Treg细胞和Treg细胞呈正相关(r=0.563,P<0.05),Treg细胞和Th17细胞呈负相关(r=-0.522,P<0.05);Treg细胞和Th17细胞和PASI无相关性(r值分别为0.021、0.076,P>0.05)。结论PV病情发展与CD4+CD25+调节性T细胞、Foxp3、IL-10和TGF-β的表达下降和Th17细胞表达上升有关。 Objective To investigate the expression and clinical significance of Th17 cells,CD4+and CD25+regulatory T cells in patients with psoriasis vulgaris(PV).Methods A total of 64 patients with PV who were admitted and treated in our hospital from June 2017 to June 2018 were enrolled as PV group,at the same time,the other 40 healthy subjects were enrolled as control group.The expression levels of Th17 cells,Foxp3^+Treg cells and Treg cells in peripheral blood were detected by direct immunofluorescence assay,which were analyzed and compared between the two groups.Results However the levels of CD4^+CD25^+Foxp3^+T cells and CD4+CD25+T cells in healthy subjects were significantly higher than those in the patients with PV at active stage and those in the patients with PV at resting stage(P<0.05).The levels of IL-10 and CD4^+CD25^+TGF-βin CD4^+CD25^+T cells in control group were significantly higher than those in PV group(P<0.05).The levels of IL-10 and CD4^+CD25^+TGF-βof CD4^+CD25^+T cells in healthy subjects were significantly higher than those in patients with PV at resting stage(P<0.05),however,the levels of Th17 cells in control group were significantly lower than those in PV group(P<0.05).Pearson correlation analysis showed that there was no correlation between PASI and CD25^+TGF-β^+T cells(P>0.05).There was a negative correlation between PASI and CD4^+CD25^+Foxp3^+T cells as well as CD25^+IL-10^+T cells(P<0.05),and there was a positive correlation between Foxp3+Treg cells and Treg cells(P<0.05).There was a negative correlation between Treg cells and Th17 cells(P<0.05).However there was nocorrelation between Treg cells and Th17 cells as well as PASI(P<0.05).Conclusion The progression of disease condition of PV is related to the decrease of the expression levels of CD4^+CD25^+regulatory T cell,Foxp3,IL-10 and TGF-βand the increase of expression levels of Th17 cell.
