Rehousing for Elderly Independent Living:Taking"Housing to Health"Project in Nottingham as an Example

在英国,许多老年人由于在医院治疗后没有适合康复的住所环境而无法按时出院,严重占用了医疗资源。针对这一问题,英国诺丁汉地区试行的"住房供给健康"计划,尝试对该类人群的住房条件进行提早干预。通过提供专业化的康复环境,在17个月内将居住在常规保障性住房中的129位老人安置于适老化的老年人公寓中,成功地缓解了康复环境与医疗环节无法顺畅对接的问题。

In the UK,the timely discharge of elderly patients occupying a highdemand bed,whose discharge is being delayed because they cannot be discharged to their current home.Delayed discharge from hospital care is a serious issue for the NHS.In order to solve this problem,Nottingham City Homes(NCH)and Nottingham CityCare Partnership propose a"Housing to Health"scheme to provide early intervention in supporting citizens affected by poor or inappropriate housing for supporting people to regain or remain independent.By providing health or other professionals in the community,129 clients were successfully re-housed into a suitable social housing property over the 17 months.The project successfully improves the links and relationships with discharge teams and other NHS staff.
