序贯机械通气对重症肺炎合并呼吸衰竭患者的治疗效果分析 被引量:8

摘要 目的探究序贯机械通气对重症肺炎合并呼吸衰竭患者的治疗效果分析。方法随机选取2016年4月-2018年4月在我院接受治疗的140例重症肺炎合并呼吸衰竭患者作为研究对象,按单双号分配法将其分为两组,参照组和观察组,每组均为70例。给予参照组患者传统有创机械通气,对观察组患者实施序贯机械通气。对临床指标、VAP发生率、再插管率及病死率及治疗前后肺功能进行对比分析。结果观察组患者的平均住院时间、总通气时间以及有创通气时间要短于参照组患者(P<0.05);在治疗前,两组患者的FEV1、FEV1%以及FEV1/FVC没有区别(P>0.05),在治疗后,观察组患者的FEV1、FEV1%以及FEV1/FVC水平要高于参照组患者(P<0.05);观察组VAP发生率、再插管率及病死率均低于参照组,比较差异有统计学意义(P<0.05)。结论在临床上,对重症肺炎合并呼吸衰竭患者采用序贯机械通气的方法,能够明显改善患者的肺功能,促进患者肺部表面活性蛋白的恢复,值得进一步推广。 Objective To explore the effect of sequential mechanical ventilation on severe pneumonia patients with respiratory failure.Methods 140 patients with severe pneumonia complicated with respiratory failure who were treated in our hospital from April 2016 to April 2018 were randomly selected as the research objects.They were divided into two groups according to the single and double number distribution method.The reference group and the observation group had 70 cases in each group.The patients in the control group were given traditional invasive mechanical ventilation and the patients in the observation group were given sequential mechanical ventilation.The clinical indicators,the incidence of V AP,the rate of re-intubation and mortality,and the pulmonary function before and after treatment were compared and analyzed.Results The average hospitalization time,total ventilation time and invasive ventilation time in the observation group were significantly shorter than those in the control group(P<0.05);before treatment,there was no significant difference in FEV1,FEV1% and FEV1/FVC between the two groups(P>0.05).After treatment,the levels of FEV1,FEV1% and FEV1/FVC in the observation group were significantly higher than those in the control group(P<0.05).The re-intubation rate and mortality rate were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Clinically,the use of sequential mechanical ventilation in patients with severe pneumonia complicated with respiratory failure can significantly improve the lung function of patients and promote the recovery of surfactant protein in the lungs of patients,which is worth further promotion.
