气压驱动泵联合依诺肝素预防骨科术后深静脉血栓的效果研究

Effects of pneumatic driven pump combined with enoxaparin on prevention of deep vein thrombosis after orthopedic surgery

目的探讨气压驱动泵联合依诺肝素预防骨科术后深静脉血栓的效果。方法选取2018年3月至2019年6月住院治疗的骨科术后患者92例,采用随机数字表法分为对照组和观察组,每组46例。对照组患者采用气压驱动泵治疗后不予以药物干预,观察组患者在对照组基础上采用依诺肝素钠预防。比较2组患者下肢深静脉血栓发生率、肺栓塞发生率等指标。结果观察组患者术后下肢深静脉血栓发生率、肺栓塞发生率明显低于对照组,差异有统计学意义(P<0.05)。观察组患者凝血反应时间、血细胞凝集成块速率、血细胞凝集成块时间明显优于对照组,差异有统计学意义(P<0.05)。观察组患者凝血参数明显优于对照组,差异有统计学意义(P<0.05)。观察组患者凝血酶时间及股静脉血流平均速度明显优于对照组,差异有统计学意义(P<0.05)。结论对骨科术后患者预防性应用依诺肝素钠及气压驱动泵,可有效降低深静脉血栓发生率,值得推广和临床应用。

Objective To investigate the effects of pneumatic driven pump combined with enoxaparin on prevention of deep vein thrombosis(DVT)after orthopedic surgery.Methods Ninety-two patients who were hospitalized for orthopedic surgery from March 2018 to June 2019 were enrolled in the study,who were divided into 2 groups,with 46 cases in each group,according to random number table method.The patients in control group were not treated by drug intervention after treatment with a pneumatic driven pump,while those in observation group,on the basis of control group,were treated by enoxaparin sodium for prevention.The incidence of DVT of lower extremities and pulmonary embolism were observed and compared between the two groups.Results The incidence rates of lower extremity DVT and pulmonary embolism in observation group were significantly lower than those in control group(P<0.05).The coagulation reaction time,hemagglutination rate,and hemagglutination time in observation group were superior to those in control group(P<0.05).Moreover the coagulation parameters,thrombin time and mean blood flow velocity of femoral vein in observation group were superior to those in control group(P<0.05).and the differences were statistically significant(P<0.05).Conclusion The prophylactic application of prosinoxaparin sodium and pneumatic driven pumps for patients after orthopedic surgery can effectively reduce the incidence of DVT.Therefore,it is worthy of popularization and clinical application.
