DCE-MRI辅助评估宫颈癌病情严重程度的应用价值研究

Application Value of DCE-MRI in Assessing the Severity of Cervical Cancer

目的探讨DCE-MRI辅助评估宫颈癌病情严重程度的效果。方法选取2016年3月至2017年10月在我院接受治疗的宫颈癌患者为研究对象所有患者均接受CT和DCE-MRI检查,观察两种检查方法对宫颈癌诊断准确率、特异度和灵敏度的差异,比较两组检查方式在宫颈癌患者病情严重程度评估中的价值。结果 DCE-MRI对宫颈癌诊断的准确率为98.33%,高于CT检查方式差异有统计学意义(χ~2=16.214,P<0.001);DCE-MRI检查对宫颈癌诊断的灵敏度、特异度、阳性预测值和阴性预测值分别为92.35、94.72、94.48、95.12,高于CT检查;DCE-MRI检查对盆腔转移、子宫体浸润和阴道浸润诊断的准确率分别为93.33%、93.33%、95.38%,明显高于CT检查的66.67%、75.00%、83.08%,差异有统计学意义(χ~2=10.000、7.566、5.123,P=0.002、0.006、0.024);DCE-MRI检查对盆腔转移、子宫体浸润和阴道浸润诊断的灵敏度、特异度、阳性预测值和阴性预测值分别为91.36、91.05、90.78、92.14,90.56、90.48、91.55、92.06,92.01、91.17、90.63、92.02,明显高于CT检查。结论 DCE-MRI检查对宫颈癌及其是否转移和浸润的诊断准确率和灵敏度较高,具有良好的术前辅助诊断价值。

Objective To investigate the effect of DCE-MRI in evaluating the severity of cervical cancer.Methods From March 2016 to October 2017 cervical cancer patients who were treated in our hospital were selected as the research subjects.All patients were examined by CT and DCE-MRI.The differences of accuracy specificity and sensitivity of two methods in the diagnosis of cervical cancer was observed,and the value of two methods in the evaluation of the severity of cervical cancer patients was compared.Results The accuracy of DCE-MRI in the diagnosis of cervical cancer was 98.33%,which was higher thanthat of CT,and the difference was statistically significant(χ~2=16.214 P<0.001).The sensitivity,specificity positive predictive value and negative predictive value respectively were 92.35,94.72,94.48,and 95.12,which were higher than those of CT.The accuracy of DCE-MRI for diagnosis of pelvic metastasis,uterine body invasion,and vaginal infiltration respectively were 93.33%,93.33%,and95.38%,and significantly higher than 66.67%,75.00%,and 83.08% of those CT,and the difference was statistically significant(χ~2=10.000、7.566、5.123,P=0.002、0.006、0.024).The sensitivity,specificity,positive predictive value and negative predictive value of DCE-MRI were 91.36,91.05,90.78,92.14,90.56,90.48,91.55,92.06,92.01,91.17,90.63,92.02,respectively,which were significantly higher than those of CT.Conclusion DCE-MRI has a high accuracy and sensitivity in the diagnosis of cervical cancer and its metastasis and invasion,and has good preoperative diagnostic value.
