摘要
目的探讨联合检测血清淀粉样蛋白A(SAA)、细胞角蛋白19片段(CYFRA21-1)、CA125、CA153在乳腺癌诊断中的意义。方法收集185例乳腺癌患者为乳腺癌组,乳腺良性肿瘤者和健康体检者各50例作为良性组和对照组,检测各组的SAA、CYFRA21-1、CA125和CA153值。使用SPSS 17. 0软件,经Mann-Whitney U检验比较各组间血清SAA、CYFRA21-1、CA125、CA153水平是否存在显著差异,运用逐步Logitic回归并绘制SAA、CYFRA21-1、CA125、CA153以及联合检测的ROC受试者工作曲线,比较各指标对乳腺癌诊断效能。结果乳腺癌组的SAA、CYFRA21-1、CA125和CA153水平高于良性组和对照组(P <0. 05),良性组和对照组之间差异无统计学意义(P> 0. 05);SAA、CYFRA21-1、CA125和CA153的ROC曲线下面积(AUC)分别为0. 758、0. 789、0. 794和0. 799,联合检测AUC为0. 897,诊断效能明显提高,优于单项检测。结论在乳腺癌诊断中,联合检测SAA、CYFRA21-1、CA125和CA153可以提高检测的准确度、灵敏度,是诊断乳腺癌的较佳选择。
Objective To investigate the significance of combined detection of serum SAA,CYFRA21-1,CA125,CA153 in the diagnosis of breast cancer. Methods A total of 185 breast cancer patients were enrolled in the breast cancer group,50 patients with benign breast tumors and 50 healthy subjects were enrolled in the benign group and the control group. The values of SAA,CYFRA21-1,CA125 and CA153 were also detected. Using the SPSS 17. 0 software,the Mann-Whitney U test was used to compare the differences of serum SAA,CYFRA21-1,CA125,CA153 levels among the groups;the stepwise Logistic regression was used to draw ROC curves for SAA,CYFRA21-1,CA125,CA153,and their combined test,so as to compare the efficacy of each indicator for breast cancer diagnosis. Results The levels of SAA,CYFRA21-1,CA125 and CA153 in breast cancer group were higher than those in benign group and control group( P < 0. 05). There was no significant difference between benign group and control group( P > 0. 05);the area under the ROC curve( AUC) of SAA,CYFRA21-1,CA125 and CA153 were 0. 758,0. 789,0. 794 and 0. 799,respectively. The combined detection of AUC was 0. 897,and the diagnostic efficiency was significantly improved,which was superior to the single test. Conclusion In the diagnosis of breast cancer,combined detection of SAA,CYFRA21-1,CA125 and CA153 can improve the accuracy and sensitivity of detection,and is a better choice for the diagnosis of breast cancer.
