摘要
目的探讨胃癌组织中微小RNA(miR)-181、miR-133a表达变化及意义。方法收集113例胃癌患者手术切除的癌组织及其癌旁正常组织样本,采用实时定量PCR技术检测组织中miR-181、miR-133a表达;分析胃癌组织中miR-181、miR-133a表达与患者临床病理特征的关系,用Kaplan-Meier生存曲线评估miR-181、miR-133a表达与患者术后5年总生存率的关系,Cox回归模型分析胃癌患者预后的影响因素。结果与癌旁正常组织相比,胃癌组织中miR-181相对表达量高,miR-133a相对表达量低(P均<0.05)。胃癌组织中miR-181表达与患者性别、年龄、病理类型无关(P均>0.05),与肿瘤直径、分化程度、浸润深度、淋巴结转移、远处转移及TNM分期有关(P均<0.05);miR-133a表达与患者年龄、性别、肿瘤直径、分化程度、病理类型、TNM分期无关(P均>0.05),与肿瘤浸润深度、淋巴结转移、远处转移有关(P均<0.05)。miR-181高表达组术后5年总生存率、中位生存时间均低于miR-181低表达组(P均<0.05)。miR-133a低表达组术后5年总生存率、中位生存时间均低于miR-133a高表达组(P均<0.05)。胃癌患者预后的危险因素有浸润深度(浆膜层)、淋巴结转移、远处转移、TNM分期较高及miR-181高表达、miR-133a低表达(HR均>1,P均<0.05)。结论胃癌组织中miR-181高表达、miR-133a低表达,二者可能参与胃癌的发生发展,且影响患者预后。
Objective To explore the expression changes and significance of microRNA(miR)-181 and miR-133a in the gastric cancer tissues.Methods The cancer tissue samples and the adjacent normal tissue samples were collected from 113 patients with gastric cancer,and the real-time quantitative PCR(RT-PCR)was used to detect the relative expression of miR-181 and miR-133a in the tissue samples.The correlation of miR-181 and miR-133a expression with the clinicopathological characteristics were analyzed.Kaplan Meier survival curve was used to evaluate the relationship between the expression of miR-181,miR-133a and the 5-year overall survival rate of patients in the gastric cancer tissues.The Cox regression model was applied to determine the prognostic factors for patients with gastric cancer.Results Compared with the adjacent normal tissues,the relative expression of miR-181 increased,while the relative expression of miR-133a decreased in the gastric cancer tissues(both P<0.05).The expression of miR-181 was not related to gender,age or pathological type(all P>0.05),but it was related to the tumor size,differentiation degree infiltration depth,lymph node metastasis,distance metastasis and the TNM stage(all P<0.05).The expression of miR-133a was not related to age,gender,tumor diameter,differentiation degree,pathological type or TNM stage(all P>0.05),but it was related to the infiltration depth,lymph node metastasis,and distance metastasis(all P<0.05).The overall 5-year survival rate and median survival time of patients with high miR-181 expression were lower than those of patients with low miR-181 expression(both P<0.05).The overall 5-year survival rate and median survival time of patients with low miR-133a expression were lower than those of patients with high miR-133a expression(both P<0.05).The risk factors affecting the prognosis of patients with gastric cancer were infiltration depth(serous layer),lymph node metastasis,distant metastasis,high TNM stage,high expression of miR-181 and low level of miR-133a(HR>1,all P<0.05).Conclusion MiR-181 is highly expressed and miR-133a is low expressed in the gastric cancer,and both may be involved in the occurrence and development of gastric cancer and affect the prognosis of patients.
