MRI内耳水成像检查对位置性眩晕的诊断价值 被引量:2

摘要 目的:研究MRI内耳水成像检查在位置性眩晕病因诊断中的临床应用价值。方法:纳入2017年8月~2018年12月在北京市大兴区中西医结合医院就诊的确诊为位置性眩晕的患者56例为观察对象,其中26例确诊为耳石脱落,12例确诊为壶腹脊病变,18例确诊为半规管病变。所有患者均接受MRI内耳水成像及颞骨高分辨CT检查。分析单纯MRI内耳水成像检查和单纯颞骨高分辨CT检查诊断位置性眩晕病因的检出准确率,比较两种检查方法的统计学差异。结果:MRI内耳水成像诊断不同位置性眩晕病因的准确率均高于颞骨高分辨CT。其中,在耳石脱落(χ2=5.026,P=0.025)和半规管病变(χ2=4.985,P=0.026)的诊断中,MRI内耳水成像准确度明显高于CT,差异具有统计学意义(P<0.05)。结论:MRI内耳水成像检查对位置性眩晕病因的诊断具有较高的参考价值。 Objective:To study the clinical value of MRI inner ear water imaging in the diagnosis of positional vertigo.Methods:The subjects were diagnosed as patients with positional vertigo from August 2017 to December 2018.Among them,26 were diagnosed as otolith shedding,12 were diagnosed as ampullary ridge disease,and 18 were diagnosed.It is a semicircular canal lesion.All patients underwent MRI inner ear water imaging and high-resolution CT examination of the tibia.The accuracy of detection of the cause of positional vertigo was analyzed by simple MRI inner ear water imaging examination and simple humeral high resolution CT examination.The statistical difference between the two methods was compared.Results:The accuracy of MRI inner ear water imaging in the diagnosis of different location vertigo was higher than that of tibia high resolution CT.Among them,in the diagnosis of otolith shedding(χ2=5.026,P=0.025)and semicircular canal lesions(χ2=4.985,P=0.026),the accuracy of MRI inner ear water imaging was significantly higher than that of CT,and the difference was statistically significant(P<0.05).Conclusion:MRI inner ear water imaging examination has a high reference value for the diagnosis of the location of vertigo.
