气管插管麻醉和喉罩全麻在异位妊娠腹腔镜手术中的应用效果对比

Comparison of the Effects of Tracheal Intubation Anesthesia and Laryngeal Mask General Anesthesia in Laparoscopic Surgery for Ectopic Pregnancy

目的:研究在异位妊娠腹腔镜手术中对比应用喉罩与气管插管进行全麻的临床麻醉效果分析。方法:选择行腹腔镜手术治疗的异位妊娠患者86例作为临床研究对象,依据患者的入院先后顺序交叉随机分组(气管插管组43例及喉罩组43例),气管插管组采取气管插管麻醉,喉罩组采取喉罩全麻。对比分析两组患者的血流动力学指标:PETCO2、PAW、MAP与HR的各时间点水平变化,同时观察两组患者的并发症发生情况并进行对比。结果:两组患者的PETCO2、PAW比较显示:气腹后的两组均高于气腹前(P<0.05),但气管插管组和喉罩组两组间比较无统计学意义(P>0.05)。两组患者的MAP与HR的各时间点水平变化比较显示:气管插管组各时间点差异有统计学意义(P<0.05),喉罩组各时点比较无统计学意义(P>0.05)。气管插管组并发症率远高于喉罩组(P<0.05)。结论:喉罩全麻与气管插管麻醉应用于腹腔镜手术中,两种方式比较喉罩全身麻醉效果更理想,且并发症发生率低,安全性更高。

Objective:To study the clinical anesthesia effect of laryngeal mask and tracheal intubation for general anesthesia during ectopic pregnancy laparoscopic surgery.Methods:86 patients with ectopic pregnancy who underwent laparoscopic surgery were selected as clinical research objects.According to the order of patients’admission,they were randomly divided into three groups(43 cases in the tracheal intubation group and 43 cases in the laryngeal mask group).Anesthesia was intubated,and the laryngeal mask group underwent general anesthesia.The hemodynamic indexes of the two groups of patients were compared and analyzed:the changes of the levels of PETCO2,PAW,MAP and HR at each time point.The complications of the two groups of patients were observed and compared.Results:The comparison of PETCO2 and PAW in the two groups showed that the two groups after pneumoperitoneum were higher than those before pneumoperitoneum(P<0.05),but there was no statistically significant difference between the two groups of tracheal intubation group and laryngeal mask group(P>0.05).The comparison of the levels of MAP and HR at each time point between the two groups of patients showed that the difference between the time points of the tracheal intubation group was statistically significant(P<0.05),and that at the time points of the laryngeal mask group was not statistically significant(P>0.05)..The complications rate in the tracheal intubation group was much higher than that in the laryngeal mask group(P<0.05).Conclusion:General anesthesia and tracheal intubation anesthesia for laryngeal masks are used in laparoscopic surgery.Compared with general laryngeal masks,general anesthesia is more effective,and the incidence of complications is lower,and the safety is higher.
