摘要
目的探讨鼻咽癌患者血清miRNA-331-3p表达水平、临床意义及对鼻咽癌筛查的价值。方法收集2017年10月至2019年2月湖南省人民医院耳鼻咽喉头颈外科诊断为鼻咽癌患者50例及健康体检者50例血清标本,采用实时荧光定量聚合酶链式反应测定血清标本miRNA-331-3p表达水平。统计分析鼻咽癌患者血清miRNA-331-3p相对表达水平与鼻咽癌临床及病理参数间的关联,采用受试者工作特征曲线分析miRNA-331-3p对鼻咽癌的诊断效率。结果①鼻咽癌组血清miRNA-331-3p相对表达量较正常组低(P<0.01)。②鼻咽癌组血清miR-331-3p相对表达量在不同TNM分期及远处转移情况中差异有统计学意义(P<0.01);在性别、年龄、肿瘤范围、淋巴结转移上差异均未见统计学意义(P均>0.05)。③血清中miR-331-3p相对表达量诊断鼻咽癌的最佳临界值为1.81,其诊断的灵敏度为74.2%,特异度为93.1%。结论鼻咽癌患者血清miRNA-331-3p表达水平出现下调,血清miRNA-331-3p与肿瘤TNM分期及远处转移有关,有较好的鼻咽癌检测效能,有助于肿瘤病情进展判断,有望成为鼻咽癌筛查的新分子标志物。
Objective To investigate the expression of serum microRNA-331-3 p levels in nasopharyngeal carcinoma(NPC) patients and its clinical significance and its diagnostic value. Methods Fifty patients with NPC and 50 healthy subjects in the People’s Hospital of Hunan Province from October 2017 to February 2019 were selected as the study subjects. Real-time polymerase chain reaction was used to determine the expression of miRNA-331-3 p in serum samples. Statistical analysis was used to investigate the relationship between serum miRNA-331-3 p level and clinical and pathological parameters of nasopharyngeal carcinoma patients. Receiver operating characteristic curve was used to analyze miRNA-331-3 p diagnostic efficiency of nasopharyngeal carcinoma. Result ①The relative expression of serum microRNA331-3 p in NPC group was lower than that in normal group(P<0.05). ②The relative expression of serum microRNA-331-3 p in nasopharyngeal carcinoma group had significant difference in different TNM stages and distant metastasis(P<0.05), but there was no significant difference in sex, age, tumor size or lymph node metastasis(P>0.05). ③The optimal critical value of the relative expression of microRNA-331-3 p in serum for the diagnosis of nasopharyngeal carcinoma was 1.81, with sensitivity of 74.2% and specificity of 93.1%. Conclusions The expression level of serum microRNA-331-3 p in patients with nasopharyngeal carcinoma is down-regulated. Serum microRNA-331-3 p is correlated with TNM stage and distant metastasis of nasopharyngeal carcinoma. It has better detection efficiency and helps to judge the progress of nasopharyngeal cancer. It is expected to become a new molecular marker for nasopharyngeal cancer screening.
