经皮微创椎弓根螺钉内固定对胸腰椎骨折患者疼痛程度及功能的影响

Effect of Percutaneous Minimally Invasive Pedicle Screw Fixation on Pain Degree and Function in Patients with Thoracolumbar Fracture

目的研究经皮微创椎弓根螺钉内固定对胸腰椎骨折患者疼痛程度及功能的影响。方法选择2018年3月-2019年1月本月收治的76例胸腰椎骨折患者,入选患者均为单纯压缩骨折,无脊髓和神经根压迫症状,即OA分型中的A1和A2型。根据随机数表法分为A、B两组,每组各38例。A组行开放性椎弓根螺钉内固定术治疗,B组行经皮微创椎弓根螺钉内固定术治疗。观察两组手术情况、疼痛程度及脊髓功能。结果B组的术中出血量、切口长度及术后6个月的VAS评分均较A组低,差异具有统计学意义(P<0.05);术后6个B组腰椎JOA评分明显高于A组,差异具有统计学意义(P<0.05)。结论对胸腰椎骨折患者实施经皮微创椎弓根螺钉内固定手术可有效减轻患者术后疼痛,术后腰椎功能恢复良好,值得应用推广。

Objective To study the effect of percutaneous minimally invasive pedicle screw fixation on pain and function of patients with thoracolumbar fracture.Methods 76 patients with thoracolumbar fractures admitted from March,2018 to January,2019 were selected.All the patients were simple compression fractures without spinal cord and nerve root compression symptoms,namely A1 and A2 in OA classification.According to the random number table method,they were divided into two groups,ie.A and B,with 38 cases in each group.Group A underwent open pedicle screw fixation,and group B underwent percutaneous minimally invasive pedicle screw fixation.The operation condition,pain degree and spinal cord function were observed.Results The intraoperative bleeding volume,incision length and VAS score of group B were lower than those of group A in 6 months postoperatively(P<0.05),and the JOA score of lumbar spine in group B was significantly higher than that of group A in 6 months postoperatively(P<0.05).Conclusion Percutaneous minimally invasive pedicle screw fixation for thoracolumbar fractures can effectively relieve postoperative pain,and the lumbar function recovers well after operation,which is worthy of application and promotion.
