基于数据挖掘技术探析韩明向教授治疗慢性咳嗽临证经验

Exploring Professor Han Mingxiang’s Clinical Experience in Treating Chronic Cough Based on Data Mining Technology

目的通过数据挖掘技术探讨韩明向教授临床辨治慢性咳嗽处方用药规律与临证心得。方法收集韩明向教授临床诊治慢性咳嗽病案650例,提取方药信息建立数据库,进行频数分析、关联规则分析、聚类分析、核心网络药物分析。结果①650首方药,共用药227味,频数11371次,其中出现频次最高的药物为百部、陈皮、紫菀、款冬花等;②核心药物关联规则分析得出韩明向教授临床治疗慢性咳嗽最常用的药对为:百部→前胡、紫菀→前胡、陈皮→茯苓、百部→款冬花等;③核心药物聚类分析共聚为13类,按功效归纳出韩明向教授临床常用健脾、温肺、润肺、清肺、解表、固表6法治疗慢性咳嗽。④核心网络药物图得出韩明向教授临床常用核心处方为:紫菀+百部+款冬花+白前+陈皮+茯苓+炙甘草;白前+陈皮+半夏+荆芥+细辛+茯苓+白术+桔梗;干姜+细辛+五味子+麻黄+苦杏仁+紫菀+款冬花+浙贝母+陈皮+炙甘草;紫菀+款冬花+百部+浙贝母+干姜+细辛+五味子+麻黄+苦杏仁。结论韩明向教授临床治疗慢性咳嗽多从温润立法,或佐以温肺化饮,或佐以疏表散寒,或佐以健脾化痰,或佐以益肺固表。

Objective Through data mining technology,to analyze the prescription rules and clinical experience of Han Mingxiang in syndrome differentiation and treatment of chronic cough.Methods A total of 650 cases of Han Mingxiang's clinical diagnosis and treatment of chronic cough were collected.The database of drug information in the medical record was extracted,and then the database was used for frequency analysis,association rule analysis,cluster analysis and core network drug analysis.Results①There were 650 prescription drugs,a total of 227 drugs,the frequency of 11371 times,and the most frequently used drugs were Radix Stemonae,Pericarpium Citri Reticulatae,Radix Asteris,Flos Farfarae,etc.②Through analysis of association rules of core drugs,the commonly used drug combinations of Professor Han's clinical treatment of chronic cough were obtained as follows:Radix Stemonae→Radix Peucedani,Radix Asteris→Radix Peucedani,Pericarpium Citri Reticulatae→Poria,Radix Stemonae→Flos Farfarae,etc.③Through cluster analysis of core drugs,13 combinations were obtained.According to the efficacy,invigorating the spleen,warming the lung,moistening the lung,clearing the lung,releasing the exterior,consolidating the exterior were commonly used in clinical treatment of chronic cough by Professor Han.④The core network drug map showed that the core prescriptions commonly used by Professor Han were:Radix Asteris+Radix Stemonae+Flos Farfarae+Rhizoma Cynanchi Stauntonii+Pericarpium Citri Reticulatae+Poria+dried Radix Glycyrrhizae;Rhizoma Cynanchi Stauntonii+Pericarpium Citri Reticulatae+Rhizoma Pinelliae+Herba Schizonepetae+Herba Asari+Poria+Rhizoma Atractylodis Macrocephalae+Radix Platycodonis;Rhizoma Zingiberis+Herba Asari+Fructus Schisandrae Chinensis+Herba Ephedrae+bitter Semen Armeniacae Amarum+Radix Asteris+Flos Farfarae+Bulbus Fritillariae Thunbergii+Pericarpium Citri Reticulatae+dried Radix Glycyrrhizae;Radix Asteris+Flos Farfarae+Radix Stemonae+Bulbus Fritillariae Thunb+Rhizoma Zingiberis+Herba Asari+Fructus Schisandrae Chinensis+Herba Ephedrae+bitter Semen Armeniacae Amarum.Conclusion Professor Han Mingxiang's clinical treatment of chronic cough is often from warming and moistening,or with warming the lung and resolving the fluids,or with releasing the exterior and dispelling the cold,or with invigorating the spleen and resolving the phlegm,or with benefiting the lung and consolidating the exterior.
