超微血管成像技术评价颈动脉斑块内新生血管与中风的相关性

摘要 目的 应用超微血管成像技术(SMI)评价动脉斑块内新生血管的情况及其与中风的相关性。方法 抽取2016年5月至2018年6月信阳市中心医院常规动脉超声检查中出现低回声动脉斑块的200例患者作为研究对象,按照病史分为有中风病史的A组(n=63)与无中风史的B组(n=137)。对两组患者均行常规超声、SMI检查及超声造影检查。分别评估各组中新生血管评分、动脉斑块内新生血管检出率。并在检查后对所有患者随访1年,统计1年内不同新生血管评分患者的中风发生率。结果 Kappa检验分析显示,超声造影及SMI检查结果具有高度一致性。超声造影评分及SMI评分2~3分中风发生率分别高于超声造影评分及SMI评分0~1分中风发生率(P<0.05)。超声造影评分0~1分中风发生率与SMI评分0~1分中风发生率比较,差异未见统计学意义(P>0.05);超声造影评分2~3分中风发生率与SMI评分2~3分中风发生率比较,差异未见统计学意义(P>0.05)。超声造影评分及SMI评分与随访1年内中风发生率均呈正相关(P<0.05)。结论 SMI技术可通过对斑块内新生血管进行评分,判断患者动脉斑块稳定性,评估中风发生风险,从而达到有效预防中风发生的目的 。 Objective To investigate the correlation between stroke and neovascularization in carotid plaque by superb micro-vascular imaging(SMI).Methods A total of 200 patients with hypoechoic artery plaque in the routine arterial ultrasound examination of Xinyang Central Hospital from May 2016 to June 2018 were selected as the study object.According to the stroke disease history,the patients were divided into group A(n=63)and group B(n=137).All patients were examined by routine ultrasound-SMI-contrast-enhanced ultrasound.The score and the detection rate of neovascularization in arterial plaques were evaluated.All patients were followed up for 1 year after the examination,and the incidence of stroke in patients with different neovascularization scores within 1 year were recorded.Results Kappa test analysis showed that the results of contrast-enhanced ultrasound and SMI were highly consistent.The incidence of stroke among patients with contrast-enhanced ultrasound score and SMI scores of 2-3 scores was higher than those of 0-1 score(P<0.05).The incidence of stroke showed no difference between contrast-enhanced ultrasound of 0-1 score and SMI of 0-1 score(P>0.05).The incidence of stroke showed no difference between contrast-enhanced ultrasound of 2-3 scores and SMI of 2-3 scores(P>0.05).Contrast-enhanced ultrasound score and SMI score were positively correlated with the incidence of stroke within 1 year of follow-up(P<0.05).Conclusions SMI technology can judge the stability of plaque and assess the risk of stroke by scoring the neovascularization in plaque,which can prevent the incidence of stroke.
