神经节苷脂治疗新生儿缺氧缺血性脑病的临床效果

摘要 目的 观察神经节苷酯治疗新生儿缺氧缺血性脑病的临床疗效以及对患儿急性期和远期神经发育的影响。方法 抽取阳泉市第三人民医院儿科2017年1月至2018年3月诊治的76例新生儿缺氧缺血性脑病(HIE)患儿为回顾分析对象,以数字法随机分为观察组(38例)与对照组(38例)。对照组患儿采取HIE常规疗法,观察组患儿在该常规治疗基础上加用GM1进行治疗。比较两组患儿治疗效果,治疗前后神经行为改善效果。两组均随访12个月,观察患儿的远期神经行为。结果 观察组患儿的治疗总有效率(97.37%)高于对照组(84.21%,P<0.05);两组患儿治疗前NBNA神经行为功能量表评分均较低(P>0.05);治疗2周后观察组患儿的NBNA量表评分高于对照组(P<0.05);观察组患儿在随访3个月、6个月和12个月时的DQ值均高于对照组(P<0.05)。结论 对于新生儿HIE患儿应用神经节苷酯治疗可取得满意效果,有助于提升临床疗效并促进患儿急性期和远期神经功能发育。 Objective To analyze the clinical efficacy of ganglioside in the treatment of neonatal hypoxic-ischemic encephalopathy(HIE)and its effect on acute and long term neurodevelopment.Methods A total of 76 neonates with HIE from January 2017 to March 2018 in the Third People’s Hospital of Yangquan were selected,and they were randomly divided into observation group(38 cases)and control group(38 cases).The children in the control group were treated with HIE routine therapy,and the children in the observation group were treated with GM1 on the basis of the routine treatment.The therapeutic effect and neurobehavioral improvement before and after treatment were compared between the two groups.Both groups were followed up for 12 months to observe the long-term neurobehavior of children.Results The total effective rate(97.37%)in the observation group was higher than that in the control group(84.21%,P<0.05).The scores of NBNA neurobehavioral function scale before treatment were lower in both groups(P>0.05).After 2 weeks of treatment,the NBNA scale score of the observation group was higher than that of the control group(P<0.05).The DQ values at 3 months,6 months and 12 months of follow-up in the observation group were significantly higher than those in the control group(P<0.05).Conclusions The application of ganglioside in the treatment of neonatal HIE can achieve satisfactory results,which is helpful to improve the clinical efficacy and promote the development of acute and long-term neurological function.
