监控高强度聚焦超声消融子宫肌瘤声像图靶组织团状强回声及整体稍强回声患者预后的对比研究 被引量:1

摘要 目的:比较高强度聚焦超声(HIFU)消融治疗子宫肌瘤时病灶出现整体稍强回声或者团状强回声对术后肌瘤吸收情况的影响。方法:2011年1月-2014年3月收治行HIFU治疗的子宫肌瘤患者93例(106个肌瘤),根据术前MRI上肌瘤的T2信号,分为低信号组与等信号组,再结合超声消融术中的监控声像图灰度变化,分为整体稍强回声与团状强回声小组。分析团状强回声、整体稍强回声与肌瘤缩小率的关系。结果:T2低信号-整体稍强回声组与团状强回声组、T2等信号-整体稍强回声组与团状强回声组在同一复查时间段的缩小率比较,差异无统计学意义(P>0.05)。T2等信号-团状强回声组与T2低信号-团状强回声组在12个月复查的缩小率比较,差异有统计学意义(P<0.05)。结论:只要达到满意的消融范围,术后相同时间的缩小率与术中出现整体稍强回声或团状强回声无明显相关性。 Objective:To compare the influence of the hyperecho mass and the whole slightly strong echo of the target tissue on the absorption of myoma after operation during high intensity focused ultrasound(HIFU)ablation for hysteromyoma.Methods:93 cases of hysteromyoma treated with HIFU,a total of 106 leiomyomas were selected from January 2011 to March 2014,according to the T2 signal of myoma on MRI before operation,they were divided into the low signal group and the equal signal group.Combined with the gray change of monitoring image during ultrasound ablation,they were divided into the overall slightly strong echo and the hyperecho mass group.We analyzed the relationship between the hyperecho mass and the whole slight strong echo and the reduction rate of myoma.Results:There was no significant difference in the reduction rate between the T2 low signal-overall slightly strong echo group and the hyperecho mass group,the T2 equal-signal overall slightly strong echo group and the hyperecho mass group.The comparison of the reduction rate of the T2 equal-signal-hyperecho mass group and the T2 low signal-hyperecho mass group in the 12-month review,the difference was statistically significant(P<0.05).Conclusion:As long as the satisfactory ablation range is achieved,the reduction rate at the same time after operation has no significant correlation with the occurrence of overall slightly strong echo or hyperecho mass.
