摘要
目的:探讨超声在继发性甲状旁腺功能亢进患者中的诊断价值,同时对其临床实验室指标进行相关性分析。方法:2017年1月-2019年2月收治维持性血液透析患者68例,分为甲状旁腺增生组和正常组,分析其甲状旁腺的检出率和各指标间的相关性。结果:68例患者中,检出甲状旁腺59例(86.8%),其中甲状旁腺增生组的血清血清甲状旁腺激素(PTH)、碱性磷酸酶(ALP)、血钙(Ca)和透析时间明显高于正常组,甲状旁腺增生组中血流丰富患者的血清PTH、ALP和血磷(P)明显高于血流不丰富患者,甲状旁腺增生组中钙化患者的ALP、Ca和透析时间明显高于未钙化患者,血清PTH>300 pg/mL的甲状旁腺增生患者明显多于血清PTH<300 pg/mL,差异均有统计学意义(P<0.05)。维持性血液透析患者的血液透析时间与血清PTH呈正相关(r=0.474,P<0.05)。结论:对维持性血流透析患者行甲状旁腺超声检查可以实时观测患者的病情进展,同时与临床指标相结合,对患者病情评估提供帮助。
Objective:To explore the value of ultrasonography in the diagnosis of secondary hyperparathyroidism,and analyze its correlation with clinical laboratory indexes.Methods:68 patients with secondary hyperparathyroidism were selected from January 2017 to February 2019,they were divided into the parathyroid hyperplasia group and the normal group,we analyzed the detection rate of parathyroid and the correlation between the indexes.Results:Of the 68 patients,59 cases(86.8%)of parathyroids were detected,the serum PTH,ALP,Ca and dialysis time of hyperplasia group were significantly higher than those of normal group.In the hyperplasia group,the serum PTH,ALP and P in the group with rich blood flow were significantly higher than those in the group without rich blood flow,the ALP,Ca and dialysis time of the calcification group were significantly higher than that of the non calcification group,patients with PTH>300 pg/mL were more than those with PTH<300 pg/mL,the differences were statistically significant(P<0.05).There was a positive correlation between hemodialysis time and serum PTH(r=0.474,P>0.05).Conclusion:Parathyroid ultrasound can be used to observe the progress of patients'condition in real time,at the same time,combined with clinical indicatorsm,providing help for patients'condition evaluation.
