Th17/Treg免疫平衡监测对全激素阻断前列腺癌治疗过程的评价

Evaluation of Th17/Treg immune balance monitoring in the treatment process of prostate cancer by total hormone blockade

目的全激素阻断治疗前列腺癌过程中对外周血辅助性T细胞17(Th17)和调节性T细胞(Treg)进行监测,探讨Th17/Treg免疫失衡与前列腺癌激素抵抗的评价。方法对全激素阻断治疗前列腺癌过程中54例发展为抵抗性的患者为治疗组,以一般状况匹配的37例激素敏感性前列腺癌作为对照组,用流式细胞术检测激素难治组和激素敏感组治疗前、后患者外周血Th17和Treg细胞比例,酶联免疫吸附试验(ELISA)检测血清IL-10和IL-17,分析Th17与IL-17的相关性和Treg与IL-10的相关性以及Th17/Treg与前列腺癌控制相关性。结果治疗组外周血Th17、IL-10、IL-17水平均高于对照组(P<0.05);治疗组Treg比例低于对照组(P<0.05)。激素抵抗性前列腺癌患者经阿比特龙加泼尼松治疗3个月后,检测外周血Th17细胞比例、Treg细胞比例、IL-10、IL-17水平比较,2组差异均无统计学意义(P>0.05)。细胞因子IL-17、IL-10变化与Th17/Treg细胞变化一致且相关。结论Th17/Treg免疫平衡与前列腺癌激素抵抗性相关,Th17/Treg免疫平衡对前列腺癌激素治疗明显影响,Th17/Treg比值变化对前列腺癌激素治疗有评价意义。

Objective To monitor peripheral blood helper T cells 17(Th17)and regulatory T cells(Treg)in the treatment of prostate cancer with total hormone blockade,and to explore the evaluation of Th17/Treg immune imbalance and prostate cancer hormone resistance.Methods In the course of total hormone blockade for prostate cancer,54 patients with resistance were included in the treatment group,and 37 patients with hormone-sensitive prostate cancer were used as the control group.Flow cytometry was used to detect the ratio of Th17 and Treg cells in the peripheral blood of patients with hormone refractory group and hormone sensitive group before and after treatment.The correlation of 17 and the correlation of Treg with IL-10 and the correlation of Th1/Treg with prostate cancer control.Results The levels of Th17,IL-10,and IL-17 in the peripheral blood of the treatment group were higher than those of the control group(P<0.05);after the treatment of hormone-resistant prostate cancer with abirirone and prednisone,the proportion of peripheral blood Th17 cells detected was significantly lower than that before treatment,while the proportion of Treg cells was significantly higher than that before treatment,with statistically significant differences(P<0.05).Changes in cytokines IL-17 and IL-10 were consistent and correlated with changes in Th17/Treg cells.Conclusion The immune balance of Th17/Treg is related to the hormone resistance of prostate cancer.The immune balance of Th17/Treg has a significant impact on the hormone therapy of prostate cancer.The change in the ratio of Th17/Treg has an evaluation significance for the hormone therapy of prostate cancer.
