摘要
目的探讨糖尿病肾病应用抗心磷脂抗体(ACLA)、抗中性粒细胞胞浆抗体(ANCA)和抗肾小球基底膜(GBM)抗体联合检测的价值。方法选取本院2016年3月-2018年11月收治的、符合纳入标准的糖尿病肾病患者60例作为研究组。同期选取71例健康体检者作为对照组。收集并分析2组患者胱抑素C、C-反应蛋白(CRP)、ACLA、ANCA、抗GBM抗体、微量白蛋白情况。结果研究组CRP、胱抑素C及微量白蛋白水平均高于对照组,差异有统计学意义(P<0.01);研究组ACLA、ANCA、抗GBM抗体的阳性率均高于对照组,差异有统计学意义(P<0.01);ACLA+ANCA+抗GBM抗体联合检测的敏感度、特异度、阳性预测值及阴性预测值均高于ACLA、ANCA及抗GBM抗体单项检测,差异有统计学意义(P<0.05)。结论ACLA、ANCA和抗GBM抗体联合检测可提高糖尿病肾病的诊断阳性率。
Objective To explore the value of combined detection of anti-cardiolipin antibody(ACLA),anti-neutrophil cytoplasmic antibody(ANCA)and anti-glomerular basement membrane(GBM)antibody in the diagnosis of diabetic nephropathy.Methods A total of 60 cases with diabetic nephropathy admitted to our hospital from March 2016 to November2018 were selected into the clinical data.In the same period,71 healthy persons were selected as control group.Cystatin C,C-reactive protein(CRP),ACLA,ANCA,anti-GBM antibody and Microalbumin were collected and analyzed.Results The levels of CRP,cystatin C and Microalbumin in the study group were higher than those in the control group,with the differences statistically significant(P<0.01).The positive rates of ACLA,ANCA and anti-GBM antibodies in the study group were higher than those in the control group,with the differences statistically significant(P<0.01).The sensitivity,specificity,positive predictive value and negative predictive value of combined detection of ACLA+ANCA+anti-GBM antibody were higher than those of single detection of ACLA,ANCA and anti-GBM antibody,with the differences statistically significant(P<0.05).Conclusion The combined detection of ACLA,ANCA and anti-GBM antibodies can improve the diagnostic rate of diabetic nephropathy.
