闭合复位联合微创钢板内固定对肱骨干粉碎性骨折患者肩肘关节功能的影响

Effect of closed reduction combined with minimally invasive plate internal fixation on shoulder and elbow joint function in patients with comminuted fracture of humeral shaft

目的探讨闭合复位联合微创钢板内固定对肱骨干粉碎性骨折患者肩肘关节功能的影响。方法选取2016年6月至2018年1月期间收治的104例肱骨干粉碎性骨折患者,利用随机数表分为两组,各52例。对照组行传统切开复位内固定治疗,观察组行闭合复位联合微创钢板内固定治疗,对比两组手术相关指标、术后恢复情况、肩肘关节功能及并发症。结果观察组术中出血量较对照组低,骨折愈合时间及住院时间较对照组短,差异有统计学意义(P<0.001);术后3个月、6个月,两组Constant-Murley、Mayo评分均较术前提高,且观察组较对照组高,差异有统计学意义(P<0.001);两组并发症发生率对比,差异无统计学意义(P>0.05)。结论闭合复位联合微创钢板内固定治疗肱骨干粉碎性骨折可有效降低术中出血量,缩短住院时间,促进骨折愈合,改善患者肩肘关节功能。

Objective To investigate the effect of closed reduction combined with minimally invasive plate internal fixation on the function of shoulder and elbow joint in patients with comminuted fracture of humeral shaft.Methods 104 patients with comminuted fracture of humeral shaft from June 2016 to January 2018 were enrolled in this study.They were divided into two groups according to random number table,with 52 cases in each group.Control group were given traditional open reduction and internal fixation,while the observation group were given closed reduction combined with minimally invasive plate internal fixation.Relevant surgical outcomes,postoperative recovery,shoulder and elbow joint functions and complications were compared between the two groups.Results The intraoperative blood loss in the observation group was less than that in the control group,and the time of fracture healing and hospitalization was shorter than that in the control group,difference was significant(P<0.001).3 months and 6 months after operation,Constant-Murley and Mayo scores increased in both groups,and those of the observation group were higher those of the control group,the difference was statistically significant(P<0.001).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Closed reduction combined with minimally invasive plate internal fixation in the treatment of comminuted humeral fractures can effectively reduce intraoperative blood loss,shorten hospital stay,promote fracture healing,and improve shoulder and elbow joint function.
