HbA1c、GSP、CRP联合检测对妊娠期糖尿病早期诊断的价值分析

Combination of HbA1c, GSP and CRP in the early diagnosis of gestational diabetes mellitus

目的:分析糖化血红蛋白、糖化血清蛋白、C-反应蛋白联合检测对妊娠期糖尿病早期诊断的价值。方法:选取本院2018年4月至2019年5月收治的68例妊娠期糖尿病的孕早期孕妇作为观察组,同期68例体检健康孕妇作为对照组。比较两组孕妇孕24-28周糖化血红蛋白(Glycated hemoglobin,Hb A1c)、糖化血清蛋白(Glycosylated serum protein,GSP)、C-反应蛋白(C-reactive protein,CRP)水平及异常率,分析HbA1c、GSP、CRP与三者联合检测妊娠期糖尿病的ROC曲线及诊断效能。结果:观察组HbA1c、GSP、CRP水平以及异常发生率均高于对照组(P<0.05)。根据ROC曲线知,Hb A1c、GSP、CRP诊断妊娠期糖尿病的AUC明显小于三者联合诊断的AUC(P<0.05)。结论:妊娠期糖尿病孕妇的Hb A1c、GSP、CRP水平会异常升高,三联合检测可以提升诊断准确率。

Objective:To analyze the value of combined detection of glycosylated hemoglobin(HbA1 c),glycosylated serum protein(GSP) and C-reactive protein(CRP) in the early diagnosis of gestational diabetes.Methods: A total of 68 cases of pregnant women with gestational diabetes admitted to our hospital from April2018 to May 2019 were enrolled in the study and assigned to as the observation group, whereas 68 healthy pregnant women admitted to our hospital in the same period were assigned to the control group. The levels and abnormal rates of HbA1 c, GSP and CRP of the two groups at 24-28 weeks of pregnancy were measured and compared. The ROC curve and diagnostic efficacy of Hb A1 c, GSP and CRP were analyzed. Results: Hb A1 c, GSP, CRP level and abnormal incidence in the observation group were higher than those in the control group(P<0.05). According to the ROC curve, the AUC of individual HbA1 c, GSP and CRP in the diagnosis of gestational diabetes was significantly lower than that in the combined diagnosis of Hb A1 c, GSP and CRP(P<0.05). Conclusion: The levels of HbA1 c,GSP and CRP in pregnant women with gestational diabetes were increased. The combination may enhance the diagnostic accuracy.
