摘要
目的分析血清基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、γ干扰素(interferon-γ,INF-γ)水平与癫痫患儿脑电图的关系。方法选择2016年1月至2018年6月华中科技大学协和深圳医院收治的64例癫痫患儿纳入观察组,随机选取同期来该院体检且与观察组患儿年龄、性别相匹配的30例健康儿童纳入对照组,采集两组儿童空腹肘静脉血,测定并比较血清MMP-9、TNF-α、INF-γ水平;监测观察组患儿脑电图,计算脑电图放电指数,分析上述因子水平与脑电图的关系。结果观察组患儿血清MMP-9、TNF-α、INF-γ水平和脑电图放电指数均显著高于对照组(均P<0.05);癫痫持续状态患儿血清MMP-9、TNF-α、INF-γ水平和脑电图放电指数均显著高于癫痫频发、非癫痫频发非持续状态患儿(均P<0.05),癫痫频发患儿血清MMP-9、TNF-α、INF-γ水平和脑电图放电指数均显著高于非癫痫频发非持续状态患儿(均P<0.05)。不同发作类型癫痫患儿血清MMP-9、TNF-α、INF-γ水平和脑电图放电指数比较差异均无统计学意义(均P>0.05)。脑电图非特异性异常波、癫痫样波患儿血清MMP-9、TNF-α、INF-γ水平均显著高于脑电图正常患儿(均P<0.05),脑电图癫痫样波患儿血清MMP-9、TNF-α、INF-γ水平均显著高于脑电图非特异性异常波患儿(均P<0.05)。癫痫患儿脑电图异常率和脑电图放电指数与血清MMP-9、TNF-α、INF-γ水平均呈正相关(均P<0.05)。结论癫痫患儿血清MMP-9、TNF-α、INF-γ水平较高,且与脑电图异常率和脑电图放电指数均呈正相关。
Objective To analyze the relationship of serum matrix metalloproteinase-9(MMP-9),tumor necrosis factor-α(TNF-α)and interferon-γ(INF-γ)levels with electroencephalogram(EEG)of children with epilepsy.Method 64 children with epilepsy admitted to Huazhong University of Science and Technology Union Shenzhen Hospital from January 2016 to June 2018 were selected as observation group,30 normal healthy children who received physical examination in that hospital during the same period and had similar age and gender with observation group were selected as control group.Fasting venous blood was collected to determine levels of serum MMP-9,TNF-αand INF-γ.The EEG of epilepsy children was monitored,and the EEG discharge index were calculated.The relationship between the above factors levels and EEG discharge index were analyzed.Result The levels of serum MMP-9,TNF-αand INF-γand the EEG discharge index of observation group were significantly higher than those of control group(all P<0.05).The levels of serum MMP-9,TNF-αand INF-γand the EEG discharge index of children with status epilepticus were significantly higher than those with frequent epilepsy and those without frequent epilepsy or status epilepticus(all P<0.05),the above indexes of children with frequent epilepsy were significantly higher than those without frequent epilepsy or status epilepticus(all P<0.05).There were no significant differences in serum MMP-9,TNF-αand INF-γlevels and the EEG discharge index in children with different seizure types(all P>0.05).The levels of serum MMP-9,TNF-αand INF-γof children with non-specific abnormal wave and epileptic wave were significantly higher than those of children with normal EEG(all P<0.05).The levels of serum MMP-9,TNF-αand INF-γof children with epileptic waves were significantly higher than those of children with non-specific abnormal waves(all P<0.05).The abnormal rate of EEG and the EEG discharge index in epilepsy children were positively correlated with the levels of serum MMP-9,TNF-αand INF-γ(all P<0.05).Conclusion The levels of serum MMP-9,TNF-αand INF-γin epilepsy children are relatively higher,and they are positively correlated with the abnormal rate of EEG and the EEG discharge index.
