导乐分娩镇痛联合全程陪伴无痛分娩在高龄产妇分娩中的应用效果

Application effect of Doula labor analgesia combined with whole-process accompanying painless labor in the delivery of elderly parturients

目的探讨导乐分娩镇痛联合全程陪伴无痛分娩在高龄产妇分娩中的应用效果。方法选取2018年1~12月于我院待产的70例初产妇作为研究对象,按随机数字表法分为观察组(36例)和对照组(34例)。观察组采用导乐分娩镇痛结合全程陪伴无痛分娩,对照组采用传统方法分娩,比较两组第一、二、三、总产程、疼痛量表(PPI)评分、抑郁自评量表(SDS)、焦虑自评量表(SAS)评分及母婴结局(产后出血、产褥感染、新生儿窒息、胎儿窘迫)。结果观察组第一、二、三及总产程短于对照组,第一、二、三产程PPI评分低于对照组,SDS、SAS评分低于对照组,差异均有统计学意义(P<0.05);观察组产后出血、产褥感染、新生儿窒息、胎儿窘迫发生率低于对照组,差异均有统计学意义(P<0.05)。结论导乐分娩镇痛结合全程陪伴无痛分娩可缩短各产程,降低产妇疼痛程度,改善产妇抑郁、焦虑等负面情绪,减少产后出血、产褥感染、新生儿窒息、胎儿窘迫等不良母婴结局发生率。

Objective To investigate the application effect of Doule labor analgesia combined with whole-process accompanying painless labor in the delivery of elderly parturients.Methods A total of 70 cases of primary mothers who were expecting to give birth in our hospital from January to December 2018 were selected as research objects,and divided into observation group(n=36)and the control group(n=34)according to random number table method.The observation group used Dolol analgesia combined with painless delivery,while the control group used traditional delivery methods.The first,second and third,total labor duration,pain scale(PPI)score,self-rating depression scale(SDS)score,self-rating anxiety scale(SAS)score and maternal outcome(postpartum hemorrhage,puerperal infection,neonatal asphyxia,fetal distress)were compared between the two groups.Results The first,second,third and total labor duration of the observation group was shorter than that of the control group,the PPI scores of the first,second and third labor duration were lower than those of the control group,and the SDS and SAS scores were lower than those of the control group,with statistical differences(P<0.05).The incidence of postpartum hemorrhage,puerperal infection,neonatal asphyxia and fetal distress in the observation group was lower than that in the control group,with statistical differences(P<0.05).Conclusion The combination of Doula labor analgesia and whole-process accompanied painless labor can significantly shorten the length of labor,reduce the pain degree,improve maternal depression,anxiety and other negative emotions,and reduce the incidence of adverse maternal and infant outcomes such as postpartum hemorrhage,puerperal infection,neonatal asphyxia and fetal distress.
