疏风解毒胶囊对慢性阻塞性肺疾病患者气道重塑指标的影响

Effect of Shufeng Jiedu Capsule on airway remodeling index of patients with chronic obstructive pulmonary disease

目的观察疏风解毒胶囊治疗慢性阻塞性肺疾病(COPD)的疗效并通过CT定量分析患者气道重塑指标的变化。方法选择2017年8月-2018年11月本院呼吸内科住院治疗的慢性阻塞性肺疾病患者共120例,随机分为对照组和研究组,各60例,治疗2个月。对照组采用常规西医疗法治疗,研究组采用疏风解毒胶囊联合常规西医疗法治疗。比较2组治疗前后症状积分、肺功能指标、血液流变学指标、CT气道重塑指标等。结果治疗后,研究组治疗后症状积分显著低于对照组(P<0.05);研究组FEV1以及FEV1/FVC均显著大于对照组(P<0.05);研究组PWV、血液流变学指标全血黏度高切、血浆黏度、红细胞压积(PCV)以及纤维蛋白原(FIB)均显著低于对照组(P<0.05);研究组平均管壁厚度(WT mean)、气道面积/总横截面积比值(WA%)、气道壁厚度/外径比值(TDR%)以及CAT评分均显著低于对照组(P<0.05)。结论疏风解毒胶囊联合西医疗法治疗痰热壅肺证COPD有助于减轻患者临床症状,改善患者肺功能,同时CT检查能够直观确切地显示COPD患者肺部病变,值得在临床中推广使用。

Objective To observe the efficacy of Shufeng Jiedu Capsule in the treatment of chronic obstructive pulmonary disease(COPD)and quantitatively analyze the changes of airway remodeling indexes in patients with CT.Methods A total of 120 patients with COPD hospitalized in the Department of Respiratory Medicine of our hospital between August 2017 and November 2018 were selected and randomly divided into control group and experimental group,60 patients in each group,and patients in the two groups were treated for two months each.The control group was treated with conventional Western medicine,while the experimental group was treated with Shufeng Jiedu Capsule combined with conventional Western medicine.The score of symptoms,pulmonary function,hemorrheology index and CT airway remodeling were compared between the two groups.Results After treatment,the score of symptom of the experimental group were significantly lower than the control group(P<0.05).FEV1 and FEV1/FVC in the experimental group were significantly higher than the control group(P<0.05).PWV,high blood viscosity,plasma viscosity,packed cell volume(PCV)and fibrinogen(FIB)in the experimental group were significantly lower than the control group(P<0.05).Wall thickness mean(WT mean),ratio of airway area to total cross-sectional area(WA%),ratio of airway wall thickness to outer diameter(TDR%)and the score of CAT in the experimental group were significantly lower than the control group(P<0.05).Conclusion Shufeng Jiedu Capsule combined with Western medicine therapy in the treatment of COPD with phlegm-heat obstructing lung syndrome is helpful to reduce the clinical symptoms of patients,improve the pulmonary function of patients,at the same time,CT examination can intuitively and accurately show the pulmonary lesions of patients with COPD,which is worth promoting in clinical use.
