叉头框转录因子M1和转移相关基因1蛋白在非小细胞肺癌中的表达及其与临床特征和预后的相关性分析 被引量:12

摘要 目的探讨叉头框转录因子M1(crosshead box transcription factor M1,FOXM1)和转移相关基因1蛋白(transfer related gene 1 protein,MTA1)在非小细胞肺癌(non-small cell lung cancer,NSCLC)中的表达及其与临床特征和预后的相关性。方法选取2012年6月—2014年2月收治的行手术治疗的86例NSCLC作为研究对象,记录患者的临床资料,检测FOXM1、MTA1表达情况,观察疾病转移、预后情况,分析FOXM1、MTA1表达与NSCLC患者临床特征的关系,采用多因素logistic回归分析观察影响NSCLC患者预后的危险因素。结果FOXM1阳性表达76例,阴性表达10例,阳性率为88.37%;MTA1高表达68例,低表达18例,高表达率为79.07%。34例生存,52例死亡,生存率39.53%。是否吸烟、肿瘤分化程度、是否发生远处转移的NSCLC患者FOXM1表达情况比较差异有统计学意义(P<0.05或P<0.01),是否吸烟、不同TNM分期、不同肿瘤直径、不同肿瘤分化程度、是否发生远处转移的NSCLC患者MTA1表达情况比较差异有统计学意义(P<0.05或P<0.01)。单因素分析显示,年龄、是否吸烟、TNM分期、肿瘤直径、肿瘤分化程度、是否发生远处转移、FOXM1及MTA1表达情况是影响NSCLC患者预后的相关因素(P<0.05或P<0.01)。多因素logistic回归分析显示,年龄≤60岁、吸烟、TNM分期Ⅲ~Ⅳ期、肿瘤直径>3 cm、肿瘤分化程度低分化、发生远处转移、FOXM1表达阳性、MTA1高表达为影响NSCLC患者预后的独立危险因素(P<0.05或P<0.01)。结论FOXM1表达阳性、MTA1高表达为影响NSCLC患者预后的不良因素,临床应提高警惕。 Objective To investigate the expression of crosshead box transcription factor M1(FOXM1)and metastasis-associated gene 1(MTA1)in non-small cell lung cancer(NSCLC)and its correlation with clinical features and prognosis.Methods A total of 86 patients with NSCLC who underwent surgery in our hospital from June 2012 to February 2014 were selected as the research subjects.The clinical data of the patients were recorded,the expression of FOXM1 and MTA1 was detected,and the metastasis and prognosis of the disease were observed.The relationship between FOXM1 and MTA1 expression and clinical features of NSCLC patients was analyzed,and multivariate logistic regression analysis was used to observe the risk factors affecting the prognosis of NSCLC patients.Results There were 76 cases with positive expression of FOXM1 and 10 cases with negative expression,and the positive expression rate was 88.37%.There were 68 cases with high MTA1 expression and 18 cases with low expression,and the high expression rate was 79.07%.Thiry-four patients survived,and 52 patients died,with a survival rate of 39.53%.There were significant differences in the expression of FOXM1 in NSCLC patients in terms of smoking history,degree of tumor differentiation,and presence or absence distant metastasis(P<0.05 or P<0.01).There were significant differences in the expression of MTA1 in NSCLC patients in terms of smoking history,different TNM staging,different tumor diameters,different degrees of tumor differentiation,and presence or absence of distant metastasis(P<0.05 or P<0.01).Univariate analysis showed that age,presence or absence of smoking history,TNM staging,tumor diameter,degree of tumor differentiation,presence or absence of distant metastasis,and FOXM1 and MTA1 expression were related factors affecting the prognosis of patients with NSCLC(P<0.05 or P<0.01).Multivariate logistic regression analysis showed that age≤60 years,presence or absence of smoking history,TNM stageⅢ-Ⅳ,tumor diameter>3 cm,poor tumor differentiation,presence of distant metastasis,positive expression of FOXM1,and high MTA1 expression were independent risk factors affecting the prognosis of patients with NSCLC(P<0.05 or P<0.01).Conclusion Positive FOXM1 expression and high MTA1 expression are adverse factors affecting the prognosis of NSCLC patients,and clinical vigilance should be raised.
