二甲双胍对肥胖型多囊卵巢综合征不孕患者血清雌激素水平及妊娠率的影响

Effect of metformin on serum estrogen level and pregnancy rate of obese patients with PCOS combined with infertility

目的探讨二甲双胍对肥胖型多囊卵巢综合征(PCOS)不孕患者血清雌激素水平及妊娠率的影响。方法选取2017年5月-2018年5月该院收治的150例肥胖型PCOS不孕患者,随机分为两组,对照组进行炔雌醇环丙孕酮治疗,研究组进行二甲双胍治疗。比较两组患者血脂、TC、TG、HDL-C、LDL-C水平、妊娠率与排卵率。结果治疗前,两组TC、TG、HDL-C、LDL-C水平差异无统计学意义(P>0.05);治疗后,研究组TC、TG、HDL-C、LDL-C水平低于对照组(P<0.05)。治疗前,两组血清LH、T、FSH、内脂素水平差异无统计学意义(P>0.05);治疗后,研究组血清内脂素、T、FSH、LH、水平低于对照组(P<0.05)。研究组妊娠率与排卵率高于对照组(P<0.05)。结论对肥胖型PCOS不孕患者进行二甲双胍治疗,可对血脂、TC、TG、HDL-C、LDL-C水平进行改善,同时可提高患者妊娠率与排卵率,降低不良反应发生率,安全性高,值得临床推广。

Objective To explore the effect of metformin on serum estrogen level and pregnancy rate of obese patients with polycystic ovary syndrome(PCOS)combined with infertility.Methods A total of 150 obese patients with PCOS combined with infertility treated in the First Hospital of Jiaxing City from May 2017 to May 2018 were selected and randomly divided into two groups,the patients in control group were treated by ethinylestradiol cycloprogesterone,the patients in study group were treated by metformin.The levels of blood lipids,TC,TG,HDL-C,LDL-C,pregnancy rates,and ovulation rates in the two groups were compared.Results Before treatment,there was no statistically significant difference in TC,TG,HDL-C,and LDL-C levels between the two groups(P>0.05);after treatment,TC,TG,HDL-C,and LDL-C levels in study group were statistically significantly lower than those in control group(P<0.05).Before treatment,there was no statistically significant difference in serum LH,T,FSH,and visfatin levels between the two groups(P>0.05);after treatment,visfatin,T,FSH,and LH levels in study group were statistically significantly lower than those in control group(P<0.05).Pregnancy rate and ovulation rate in study group were statistically significantly higher than those in control group(P<0.05).Conclusion Metformin can improve the levels of blood lipids,TC,TG,HDL-C,LDL-C of obese patients with PCOS combined with infertility,raise pregnancy rate and ovulation rate,reduce the incidence rate of adverse reactions,the safety is high,which is worthy of clinical promotion.
