摘要
目的探讨γ干扰素(IFN-γ)基因多态性在子痫前期患者中的表达及对妊娠结局的影响。方法选择2017年6月至2018年10月承德市中心医院子痫前期患者71例为试验组,选择同期健康孕检的妊娠患者69例为对照组。两组入院后均取外周静脉血3mL,血清分离后采用单碱基延伸法PCR扩增IFN-γ基因SNP位点;利用单碱基延伸法分析IFN-γ基因5个SNP位点的基因型;采用酶联免疫吸附试验测定两组不同基因型下IFN-γ水平;记录两组胎死宫内、新生儿窒息、新生儿透明膜病、肝功能损害、急性心衰及DIC发生率。结果试验组子痫前期患者SNP基因位点rs2069705、rs1861493、rs2193050及rs2069718基因频率比较,差异无统计学意义(P>0.05);试验组子痫前期患者SNP基因位点rs2430561中AA、AT基因频率低于对照组(P<0.05),TT基因频率高于对照组(P<0.05);试验组rs2430561中A等位基因频率低于对照组(P<0.05),T等位基因频率高于对照组(P<0.05);试验组TT、AT、AT+AA基因位点下IFN-γ水平高于对照组(P<0.05);试验组子痫前期患者胎死宫内、新生儿窒息、新生儿透明膜病、肝功能损害、急性心衰及DIC发生率均高于对照组(P<0.05)。结论IFN-γ基因中rs2430561位点是子痫前期的易感基因位点,并且不同基因型下患者血清IFN-γ均呈高表达,能影响患者妊娠结局。
Objective To investigate the expression of theγ-interferon(IFN-γ)gene polymorphism in patients with preeclampsia and its effect on the pregnancy outcome.Methods A total of 71 patients with preeclampsia treated in Chengde Central Hospital from June of 2017 to October of 2018 were selected into the experiment group,and the other 69 healthy pregnant women undergoing the pregnancy test over the corresponding period were selected into the control group.3 mL peripheral venous blood was taken from the subjects of the two groups after admission.After serum separation,the SNP site of IFN-γgene was amplified by the single base extension PCR method.The genotype of 5 SNP sites of IFN-γgene was analyzed by the single base extension method.The IFN-γlevels in two different genotypes were determined by enzyme-linked immunosorbent assay(ELISA).The incidences of fetal intrauterine death,neonatal asphyxia,neonatal hyaline membrane disease,liver damage,acute heart failure and DIC were recorded.Results The gene frequencies of the SNP gene loci rs2069705,rs1861493,rs2193050 and rs2069718 in patients with preeclampsia was not significantly different(P>0.05).The gene frequency of the SNP gene locus s2430561 was significantly higher in the experiment group than in the control group(P<0.05),that of A allele at rs2430561 was significantly lower in the experiment group than that in the control group(P<0.05)and that of T allele at rs2430561 was significantly higher in the experiment group than in the control group(P<0.05).The IFN-γlevels at the gene loci TT,AT and AT+AA were significantly higher in the experiment group than in the control group(P<0.05).The incidences of fetal intrauterine death,neonatal asphyxia,neonatal hyaline membrane disease,liver damage,acute heart failure and DIC were significantly higher in the experiment group than in the control group(P<0.05).Conclusion The gene locus rs2430561 in theγ-interferon gene is a preposed susceptibility gene locus and the serumγ-interferon is highly expressed in patients with TT genotype,which can affect the pregnancy outcome.
