超声乳化白内障吸除联合人工晶状体植入对糖尿病性白内障角膜内皮损伤的研究

Study on phacoemulsification combined with intraocular lens implantation in treatment of diabetic cataract with corneal endothelium injury

目的 探讨超声乳化白内障吸除联合人工晶状体植入对糖尿病性白内障角膜内皮损伤的情况。方法 回顾性分析2018年5月—2019年6月我院收治66例白内障患者的临床资料,根据患者有无糖尿病分为对照组(无糖尿病,n=33)和观察组(有糖尿病,n=33)。2组均实施超声乳化白内障吸除联合人工晶状体植入术,比较2组术后角膜内皮细胞损伤情况。结果 观察组术后1周角膜内皮细胞密度、六边形细胞比例低于对照组,变异系数、中央角膜厚度高于对照组,内皮细胞面积低于对照组,内皮细胞丢失率高于对照组,差异均有统计学意义(P<0.05)。结论 糖尿病性白内障患者超声乳化白内障吸除联合人工晶状体植入术后角膜内皮损伤较普通白内障患者程度严重,临床治疗应加强对角膜内皮的保护,减轻损伤。

Objective To investigate the effect of phacoemulsification combined with intraocular lens implantation on corneal endothelium cell injury in patients with diabetic cataract.Methods The clinical data of 66 patients with cataract in the hospital from May 2018 to June 2019 were retrospectively analyzed.All patients were divided into control group(without diabetes,n=33)and observation group(with diabetes,n=33)according to whether had diabetes.Two groups underwent phacoemulsification combined with intraocular lens implantation.The postoperative corneal endothelium cell injury of two groups was compared.Results After surgery for 1 week,the corneal endothelial cell density and the proportion of hexagonal cells in observation group were lower than those in control group,the variable coefficient and central corneal thickness in observation group were higher than those in control group,the area of endothelial cells in observation group was lower than that in control group,and the loss rate of endothelial cells in observation group was higher than that in control group(P<0.05).Conclusion The postoperative corneal endothelium injury in patients with diabetic cataract is more severe than that in patients with common cataract.Therefore,clinical treatment should be strengthened to protect corneal endothelium and reduce damage.
