摘要
目的分析依托咪酯复合瑞芬太尼对老年无痛胃镜检查患者血流动力学及不良反应的影响。方法 100例行无痛胃镜检查的老年患者随机分为两组各50例。观察组采用依托咪酯+瑞芬太尼麻醉,对照组采用丙泊酚+瑞芬太尼麻醉,比较两组患者检查前、检查中及检查后的血流动力学指标及不良反应。结果检查前,两组患者的血流动力学指标(MAP、HR及SpO2)比较均无统计学差异(P>0.05)。检查中,对照组患者的MAP、SpO2较检查前降低,HR升高,差异有统计学意义(P<0.05);检查后各指标逐渐恢复。观察组患者检查中的MAP、SpO2、HR与检查前比较,差异无统计学意义(P>0.05);检查后各指标逐渐恢复。两组患者检查中的MAP、SpO2、HR比较,差异有统计学意义(P<0.05)。观察组患者的心律失常和注射痛发生率显著低于对照组(P<0.05)。结论依托咪酯复合瑞芬太尼麻醉对老年无痛胃镜检查患者血流动力学的影响较小,不良反应发生率较低。
Objective To investigate the effect of etomidate combined with remifentanil on hemodynamics and adverse reactions in elderly patients with painless gastroscopy examination.Methods 100 elderly patients with painless gastroscopy examination were randomly divided into two groups, with 50 cases in each group. The observation group was anesthetized with etomidate + remifentanil, and the control group was anesthetized with propofol + remifentanil. The hemodynamic parameters before examination, during examination and after examination, and the adverse reactions were compared between two groups.Results Before examination, no significant difference was found in hemodynamic parameters(MAP, HR, and SpO2) between two groups(P>0.05). During examination, the control group had significantly decreased MAP and SpO2 and increased HR compared with those before examination(P<0.05);The indicators gradually recovered after examination. Compared with those before examination, the MAP, SpO2, and HR during examination of the observation group had no statistical differences(P>0.05);The indicators gradually recovered after examination. There was statistically significant differences in MAP, SpO2, and HR during examination between two groups(P<0.05). The incidences of arrhythmia and injection pain of observation group were significantly lower than those of control group(P<0.05).Conclusions Etomidate combined with remifentanil for anesthesia has less effect on hemodynamics and lower incidence of adverse reactions in elderly patients with painless gastroscopy examination.
