高黏度骨水泥弥散程度对老年骨质疏松椎体压缩性骨折患者临床疗效的影响 被引量:3

摘要 目的探讨高黏度骨水泥弥散程度对老年骨质疏松椎体压缩性骨折患者临床疗效的影响。方法回顾性分析2017年1月至2018年3月在我院行高黏度骨水泥椎体成形手术治疗的86例老年骨质疏松椎体压缩性骨折患者的临床资料,根据术后X线片将骨水泥弥散至上下终板两侧的43例患者纳入观察组,其他43例患者纳入对照组,比较两组患者的临床指标、伤椎高度压缩率及局部后凸角。结果观察组患者的手术时间、透视次数、骨水泥注射量均显著低于对照组(P<0.05)。治疗后,观察组患者的椎体前缘高度压缩率、中线高度压缩率、后缘高度压缩率、局部后凸角度均显著低于对照组(P<0.05)。结论高黏度骨水泥的弥散程度可影响老年骨质疏松椎体压缩性骨折患者的治疗效果,当骨水泥均匀分布于上下终板时,受伤椎体的强度和刚度得到有效提升,可减少骨水泥注射量,促进椎体恢复,有利于改善患者的病情。 Objective To explore the effect of high viscosity bone cement dispersivity on the clinical efficacy of elderly patients with osteoporotic vertebral compressive fracture(OVCF). Methods The clinical data of 86 elderly patients with OVCF who underwent vertebroplasty with high viscosity bone cement in our hospital from January 2017 to March 2018 were analyzed retrospectively. According to the postoperative X-ray film, 43 patients with bone cement diffusion on both sides of the upper and lower endplates were included in the observation group, and other patients were included in the control group. The clinical indicators, the high compression rate of the injured vertebral body and the local kyphosis angle were compared between two groups.Results The operation time, the number of fluoroscopy,and the injection amount of bone cement in the observation group were significantly lower than those in the control group(P<0.05). After treatment, the compression rate of anterior vertebral body height, compression rate of midline vertebral body height, compression rate of posterior vertebral body height, and local kyphosis angle of observation group were significantly lower than those of control group(P<0.05).Conclusions The dispersivity of high viscosity bone cement can affect the treatment effect of elderly patients with OVCF. When bone cement is evenly distributed on the upper and lower endplates, the strength and stiffness of the injured vertebral body can be effectively improved,which can reduce the injection amount of bone cement, promote vertebral body recovery and help improve patients’ disease condition.
