血浆RUNX3基因启动子甲基化检测对宫颈癌早期诊断的价值研究

Study on the value of plasma RUNX3 gene promoter methylation detection in the early diagnosis of cervical cancer

目的探究血浆人类Runt相关转录因子3(RUNX3)基因启动子甲基化对早期宫颈癌的诊断价值。方法选取80例宫颈癌患者作为A组,选取同期80例宫颈上皮内瘤变(CIN)患者作为B组,另选取80例正常宫颈组织切除者作为C组。对三组研究对象的宫颈癌组织、CIN组织、正常宫颈组织及血浆RUNX3启动子甲基化应用甲基化特异性聚合酶链反应(MCP)进行检测。比较三组组织、血浆RUNX3启动子甲基化异常率,分析宫颈癌病理特征与RUNX3启动子甲基化表达及RUNX3蛋白表达的关系。结果A组组织RUNX3启动子甲基化异常率为86.25%,血浆RUNX3启动子甲基化异常率为73.75%;B组组织RUNX3启动子甲基化异常率为33.75%,血浆RUNX3启动子甲基化异常率为26.25%;C组组织RUNX3启动子甲基化异常率为3.75%,血浆RUNX3启动子甲基化异常率为0;A组组织RUNX3启动子甲基化异常率和血浆RUNX3启动子甲基化异常率均高于B组和C组,差异有统计学意义(P<0.05)。血浆、组织RUNX3启动子甲基化异常与临床分期、肿瘤分化程度、病理类型、淋巴转移存在相关性(P<0.05)。宫颈癌患者RUNX3蛋白异常表达与淋巴转移、肿瘤分化程度、临床分期存在相关性(P<0.05)。结论血浆RUNX3基因启动子甲基化异常可提示宫颈癌的发生发展,为早期诊断宫颈癌提供新方法。

Objective To investigate the diagnostic value of plasma human runt-rela tedtran-scription factor 3(RUNX3)gene promoter methylation detection in the early diagnosis of cervical cancer.Methods There were 80 cases of cervical cancer patients selected as group A,80 cases of cervical intraepithelial neoplasia(CIN)patients at the same period selected as group B,and 80 cases of normal cervical tissue resection selected as group C.Cervical cancer tissues,CIN tissues,normal cervical tissues,and plasma RUNX3 promoter were methylated using methylation-specific polymerase chain reaction(MCP)in three groups.The abnormal methylation rate of RUNX3 promoter in tissue and plasma was observed and compared among three groups.The correlation between the pathological characteristics of cervical cancer and the expression of RUNX3 promoter methylation and RUNX3 protein was analyzed.Results The abnormal methylation rate of RUNX3 promoter in tissue of group A was 86.25%,the abnormal methylation rate of RUNX3 promoter in plasma was 73.75%;the abnormal methylation rate of RUNX3 promoter in tissue of group B was 33.75%,the abnormal methylation rate of RUNX3 promoter in plasma was 26.25%;the abnormal methylation rate of RUNX3 promoter in tissue of group C was 3.75%,the abnormal methylation rate of RUNX3 promoter in plasma was 0.The abnormal methylation rate of RUNX3 promoter in tissue and plasma of group A was higher than those of group B and group C,and their difference was statistically significant(P<0.05).The abnormal methylation of RUNX3 promoter in plasma and tissue was correlated with clinical stage,tumor differentiation,pathological type and lymph metastasis(P<0.05).Abnormal expression of RUNX3 protein in cervical cancer was related to lymph node metastasis,tumor differentiation and clinical stage(P<0.05).Conclusion Early diagnosis of abnormal RUNX3 promoter methylation in plasma can prompt the occurrence and development of cervical cancer,and provide a new method for early diagnosis of cervical cancer.
