摘要
目的 探讨辅助性Th17和调节性T细胞(Treg细胞)在湿疹发病中的作用。方法 选取湿疹患者83例,其中急性期35例,亚急性期23例,慢性期25例。流式检测患者外周血中CD3^+CD8-T细胞(即CD3^+CD4^+T细胞)白细胞介素(IL)-17A表达,以及CD4^+CD25hiFoxp3^+Treg细胞水平,并与42名健康对照相比较;选取15例急性湿疹患者皮损及10名健康对照皮肤,分离出淋巴细胞后进行流式检测,比较两者皮肤中Th17和Treg细胞的表达。结果 湿疹患者外周血Th17细胞显著高于健康对照(P<0.01),并且急性期患者Th17细胞比例高于亚急性期和慢性期患者(P<0.05);湿疹患者外周血Treg细胞和健康对照相比无明显异常;15例急性湿疹患者皮损中Th17细胞显著高于健康对照(P<0.01),并且皮损中存在Treg细胞。结论 外周血Th17细胞升高可能与湿疹发病有关,Th17和Treg细胞在湿疹皮损处参与致病。
Objective To investigate the role of Th17 and(regulatory T cells)Treg cells in patients with eczema.Methods Peripheral blood mononuclear cells were prepared from 83 patients with eczema which includes 35 cases of acute stage,23 cases of subacute stage and 25 cases of chronic stage,as well as 42 normal controls.The frequencies of Th17 cells(CD3^+CD8-IL-17A^+equal to CD3^+CD4^+IL-17A^+)and Treg cells(CD4^+CD25hiFoxp3^+)were determined by flow cytometry.The expression of Th17 and Treg cells in skin of 15 patients with acute eczema and 10 normal controls were compared by flow cytometry after lymphocyte isolation.Results Th17 cells in peripheral blood of patients with eczema were significantly higher than those of normal controls(P<0.01),and the proportion of Th17 cells in patients with acute stage was higher than that in patients with subacute and chronic stage(P<0.05).There was no difference between Treg cells in peripheral blood and normal controls.Th17 cells in skin lesions of 15 patients with acute eczema were significantly higher than those of normal controls(P<0.01),and there were Treg cells in skin lesions.Conclusion The increase of Th17 cells in peripheral blood may be related to the pathogenesis of eczema.Th17 and Treg cells are involved in the pathogenesis in eczema lesions.
