苯扎溴铵联合双氧水及氟康唑滴耳液治疗真菌性外耳道炎疗效分析

Effect of benzalkonium bromide combined with hydrogen peroxide and fluconazole ear drops in the treatment of otitis externa mycotica

目的 探析苯扎溴铵联合双氧水及氟康唑滴耳液治疗真菌性外耳道炎的临床疗效。方法 选取我院耳鼻喉科2017年3月至2019年3月诊断为真菌性外耳道炎的患者74例,按随机数字法分为观察组和对照组(各37例)。观察组患者采用苯扎溴铵联合双氧水及氟康唑滴耳液治疗,对照组采用双氧水及氟康唑滴耳液治疗,共治疗5个疗程,比较2组患者的临床总有效率、真菌转阴率,并记录不良反应,并考察2组半年内的复发率。结果 观察组临床总有效率为95%,显著高于对照组49%(P<0.05)。治疗后,观察组患者外耳道内出现曲霉菌、白假丝酵母菌、青霉菌等真菌的例数明显降低,其真菌转阴率为79%,显著高于对照组51%(P<0.05)。观察组治疗中不良反应发生率为8%,对照组为27%,观察组明显低于对照组(P<0.05);同时观察组半年内复发率显著低于对照组(P<0.05)。结论 苯扎溴铵联合双氧水及氟康唑滴耳液治疗真菌性外耳道炎临床疗效突出,不良反应及复发率降低,具有一定推广价值。

Objective To investigate the clinical efficacy of benzalkonium bromide combined with hydrogen peroxide and fluconazole ear drops in the treatment of otitis externa mycotica.Methods From March 2017 to March 2019,seventy-four patients with otitis externa mycoticawere selected and randomly divided into observation group(n=37)and control group(n=37).The patients in the observation group were treated with benzalkonium bromide combined with hydrogen peroxide and fluconazole ear drops,while the control group were treated with hydrogen peroxide and fluconazole ear drops only for a total of five courses of treatment.The total clinical effective rate,fungal negative conversion rate and adverse reactions were compared between the two groups,and the recurrence rate within half a year was investigated.Results The total effective rate of the observation groupwas 95%,which was significantly higher than that of the control group(49%)(P<0.05).After treatment,the number of fungi such as Aspergillus,Candida albicans and Penicillium in the external auditory canal of the observation group was significantly decreased,and the negative conversion rate of the fungi was 79%,which was significantly higher than that of the control group(51%)(P<0.05).The incidence of adverse reactions was 8% in the observation group and 27% in the control group,which was significantly lower than that in the control group(P<0.05),and the recurrence rate in the observation group was significantly lower than that in the control group within half a year(P<0.05).Conclusion The clinical effect of benzalkonium bromide combined with hydrogen peroxide and fluconazole ear drops in the treatment of otitis externa mycotica is prominent,the adverse reactions and recurrence rate are reduced,so it has certain popularization value.
