摘要
目的探讨联合应用血清Mac-2结合蛋白(Mac-2BP)与岩藻糖基化触珠蛋白(FUC-HPT)对慢性肝炎患者病情严重程度的诊断价值。方法选择该院2016年8月至2018年8月收治的143例慢性肝炎患者为研究对象,患者接受肝组织病理活检判断肝纤维化程度,检测患者血清Mac-2BP和FUC-HPT水平。分析所有患者肝组织病理活检结果,比较不同肝纤维化程度患者血清Mac-2BP和FUC-HPT水平,分析两种蛋白与肝纤维化程度的相关性,计算两种蛋白诊断肝纤维化程度的截断值,比较两种蛋白单用以及联合应用对慢性肝炎病情严重程度的诊断价值。结果患者血清Mac-2BP和FUC-HPT水平随着肝纤维化程度加重而上升,且上述两种蛋白在不同肝纤维化程度患者血清中的水平差异均有统计学意(P<0.05);Spearman相关分析结果显示,Mac-2BP、FUC-HPT与肝纤维化程度均为正相关关系(r=0.614、0.442,P<0.05);对肝纤维化≥S2、≥S3与S4级进行诊断时,两种蛋白联合应用的灵敏度和特异度优于单项检测结果。结论Mac-2BP和FUC-HPT可用于慢性肝炎病情严重程度的预测,二者联合应用诊断效率更高,其诊断灵敏度与特异度均优于单项指标结果。
Objective To investigate the diagnostic value of combined use of serum Mac-2 binding protein(Mac-2BP)and fucosylated haptoglobin(FUC-HPT)level for the severity in the patients with chronic hepatitis.Methods A total of 143 patients with chronic hepatitis treated in the hospital from August 2016 to August 2018 were selected as the study subjects.All cases received the liver tissue pathological biopsy for judging the degree of liver fibrosis.The levels of serum Mac-2BP and FUC-HPT were detected.The liver tissue pathological biopsy results of all cases were analyzed.The levels of serum Mac-2BP and FUC-HPT were compared among the patients with different degrees of liver fibrosis.The correlation between the two kinds of proteins and the degree of liver fibrosis was analyzed.The cut-off values of the two kinds of proteins for diagnosing the degree of liver fibrosis were calculated.The diagnostic values for the severity of chronic hepatitis were compared between the single use of these two proteins and their combined use.Results The levels of serum Mac-2BP and FUC-HPT were increased with the aggravation of the liver fibrosis severity,moreover the levels of these two kinds of proteins had statistical difference among the patients with different liver fibrosis degrees(P<0.05).The Spearman correlation analysis results showed that Mac-2BP and FUC-HPT were positively correlated with liver fibrosis(r=0.614,0.442,P<0.05).In diagnosing hepatic fibrosis level ≥S2,≥S3 and S4,the sensitivity and specificity of the combined use of the two kinds of proteins were better than those of single use.Conclusion Mac-2BP and FUC-HPT can be used to predict the severity of chronic hepatitis,and the diagnostic efficiency of the both combination use is higher.Its sensitivity and specificity are better than those of single indicator.
