经胸乳入路腔镜手术治疗甲状腺乳头状癌的效果

Effect of endoscopic thyroidectomy via breast-thoracic approach on papillary thyroid cancer

目的分析经胸乳入路腔镜手术治疗甲状腺乳头状癌(PTC)患者的效果。方法选择我院62例PTC患者,将患者按手术方式分为对照组与观察组,各31例。观察组行经胸乳入路腔镜手术治疗,对照组行开放手术治疗。比较两组患者的疗效。结果观察组患者的VAS评分、短期并发症总发生率低于对照组(P<0.05);术后1 d,观察组CRP、MDA水平低于对照组(P<0.05);术后7 d,两组CEA、CA15-3水平比较,差异无统计学意义(P>0.05);术后,观察组QLQ-52评分、PSAS、OSAS评分低于对照组(P<0.05)。结论经胸乳入路腔镜手术术治疗PTC具有疼痛少、应激反应轻、并发症少、术后美观度及生存质量更好等优势,值得临床推广。

Objective To analyze the effect of endoscopic thyroidectomy via breast-thoracic approach on patients with papillary thyroid cancer(PTC).Methods A total of 62 patients with PTC in our hospital were selected and divided into control group and observation group according to the operation modes,with 31 cases in each group.The observation group was treated with endoscopic thyroidectomy via breast-thoracic approach,while the control group was treated with open surgery.The efficacy were compared between the two groups.Results The VAS score and the total incidence of short-term complications in the observation group were lower than those in the control group(P<0.05).At 1 day after operation,the levels of CRP and MDA in the observation group were lower than those in the control group(P<0.05) at 7 days after operation,there were no significant differences in the levels of CEA and CA15-3 between the two groups(P>0.05).After operation,the QLQ-52,PSAS and OSAS scores in the observation group were lower than those in the control group(P<0.05).Conclusion The endoscopic thyroidectomy via breast-thoracic approach in the treatment of PTC has the advantages of less pain,less stress response,less complications,better postoperative aesthetics and quality of life,which is worth clinical promotion.
