恶性胸腔积液中药外治方剂组方规律

The Prescription Rule of External Treatment with Chinese Medicine for Malignant Pleural Effusion

目的:探索中药外治方剂治疗恶性胸腔积液的组方用药规律。方法:检索万方、中国知网、维普及中国生物医学文献数据库建库至今所收录的关于中医外治恶性胸腔积液的相关文献,根据纳入与排除标准进行文献筛选,并提取文献中处方。建立数据库,然后将数据录于中医传承辅助系统软件(TCMISS V2.5)中,基于复杂系统熵聚类算法、Apriori算法及无监督的熵层次聚类核心算法,获取恶性胸腔积液外治方剂组方用药规律及新处方,然后根据结果绘制相应网络图。结果:纳入方剂共57首,所关联中药共163味,总频次共849次。其中运用频次≥5次的中药共50味。进一步分析相关药物使用频率,得出了药物之间的关联规则,并挖掘出常用药对196对、外用新方核心组合12个和新方6首。结论:中医外治恶性胸腔积液以活血化瘀药、泻下药、解表药、补虚药、化痰止咳平喘药为处方之首选,并结合兼证辅以清热药、理气药、温里药等。

Objective: To explore the medication regularity and discover new prescriptions for the external treatment of malignant pleural effusion with external prescriptions of traditional Chinese medicine. Methods: The author retrieved the relevant literatures on the external treatment of malignant pleural effusion in TCM collected from the database of Wanfang, CNKI, VIP and China Biology Medicine disc, screened the literatures according to the inclusion and exclusion criteria, extracted the prescriptions in the literatures, and established the database, then recorded the data in TCMISS V2.5. Based on the three algorithms of the platform(complex system entropy clustering algorithm, Apriori algorithm and unsupervised entropy hierarchical clustering core algorithm), the suthor obtained the prescription medication rule of malignant pleural effusion and find new prescriptions, and then drew the corresponding network graph according to the results. Results: A total of 57 prescriptions were included, and 163 Chinese medicines were related, with a total frequency of 849, among which 50 herbs were used more than 5 times. The frequency of drug use was further analyzed, and the association rules between drugs were obtained. 196 pairs of commonly used drugs, 12 new core combinations for external application and 6 new prescriptions were excavated. Conclusion: Chinese medicine for external treatment of malignant pleural effusion is the first choice of prescriptions for blood-activating and stasis-removing drugs, purging drugs,relieving drug, tonic drugs, resolving phlegm, relieving cough and relieving asthma drugs. Combined with concurrent syndromes, supplemented by antipyretic drugs, Qi-regulating drugs, drugs of warming the internal and so on.
