甲状腺功能正常的2型糖尿病患者游离三碘甲状腺原氨酸水平与白蛋白尿的相关性 被引量:2

Connection between free triiodothyronine levels and albuminuria in type 2 diabetes mellitus patients with normal thyroid function

摘要 目的探搜在甲状腺激素检测正常范围内的2型糖尿病(T2DM)患者中游离三碘甲状腺原氨酸(FT 3)值与白蛋白尿的关联性。方法搜集安徽中医药大学第一附属医院收治的166例T2DM患者病历资料,按照尿白蛋白/肌酐比≥30 mg/g定义为白蛋白尿,所有患者分成白蛋白尿组55例,非白蛋白尿组111例,分析两组患者一般资料,同时采用logistic回归剖析FT 3与白蛋白尿之间的关联性。结果白蛋白尿的检出率为33%。与非白蛋白尿组相比,白蛋白组中年龄、糖尿病病程、治疗年限、收缩压、腰围、臀围、肌酐、尿酸、空腹血糖、超氧化物歧化酶、FT 3均有显著差异(P<0.05)。logistic回归剖析显示:FT 3与白蛋白尿患病危险呈负相关,未纠正任何因素时P值为0.026,而纠正性别、年龄、病程、超氧化物歧化酶等混杂因素后P值为0.027。结论FT 3值下降是白蛋白尿患病的独立风险要素,尤其是在甲状腺激素检测非异常的T2DM人群中。 Objective To explore the connection between the value of free triiodothyronine(FT 3)and albuminuria in type 2 diabetes mellitus(T2DM)patients within the normal range of thyroid hormone detection.Methods 166 T2DM patients who were received and cured to our hospital were gathered.In terms of urinary albumin/creatinine ratio≥30 mg/g,albuminuria was defined.All patients were spanided into albuminuria group(55 cases)and non-albuminuria group(111 cases).Physical data of two groups of patients were analyzed,at the same time,logistic regression was used to analyze the relevance between FT 3 and albuminuria.Results The detection rate of albuminuria was 33%.Age,systolic blood pressure,duration of diabetes,curative time,waistline,hipline,fasting blood glucose,superoxide dismutase,creatinine,unic acid and FT 3 in albumin uria group were significantly different compared with non-albuminuria group(P<0.05).Logistic regression analysis demonstrated that FT 3 was negatively correlated with the risk of albuminuria.The P value was 0.026 when no factors were corrected,while 0.027 after correcting the confounding factors such as sex,age,duration of diabetes,superoxide dismutase,etc.Conclusion The decrease of FT 3 value is an independent risk factor for albuminuria,especially in T2DM population with normal thyroid hormone detection.
