摘要
目的通过分析输注不同剂量的悬浮红细胞后受血者血钙浓度变化及低钙血症发生情况,探讨悬浮红细胞输注量对受血者血钙浓度变化的影响。方法选择2016年5月~2019年1月我院150例输血患者作为研究对象,将24 h内输血量>18 U的50例设为大量输血组,24 h内输血量为1~9 U的50例设为常规输血组1,24 h内输血量10~17 U的50例设为常规输血组2。分别检测各组输血前、输血后和输血24 h后血清钙离子水平,比较各组血清钙水平及低钙血症的发生率。结果大量输血组和常规输血组2输血后、输血24 h后血清钙离子水平均低于输血前(P<0.05);但输血后24 h血清钙离子水平高于输血后(P<0.05)。大量输血组低钙血症及重度低钙血症发生率高于常规输血组1和常规输血组2(P<0.05)。结论输血后患者血清钙离子水平随悬浮红细胞输注量增加而降低,大量输血患者低钙血症发生率较高,需在输血过程中密切监测钙离子水平。
Objective To analyze the changes of blood calcium concentration and the occurrence of hypocalcemia in recipients after transfusion of different doses of suspended red blood cells,and to explore the effect of suspended red blood cell infusion on the changes of blood calcium concentration in recipients.Methods From May 2016 to January 2019,patients with blood transfusions in our hospital were selected as the research objects.50 patients with a blood transfusion volume of>18 U within 24 h were set as a large blood transfusion group,50 patients with blood transfusion volume of 1 to 9 U within 24 h were designated as the conventional blood transfusion group 1,and 50 patients with blood transfusion volume of 10 to 17 U within 24 h were designated as the conventional blood transfusion group 2.Serum calcium levels were measured before,after,and 24 h after transfusion in each group,and the serum calcium levels and the incidence of hypocalcemia in the three groups were compared.Results The serum calcium level in the large blood transfusion group and the conventional blood transfusion group 2 was lower than that before the transfusion(P<0.05).The serum calcium level 24 h after the transfusion was higher than that after the transfusion,but lower than before the transfusion(P<0.05).The incidence of hypocalcemia and severe hypocalcemia in the large blood transfusion group was higher than that in the conventional blood transfusion group 1 and the conventional blood transfusion group 2(P<0.05).Conclusion The serum calcium level of patients after transfusion decreases with the increase of suspended red blood cell transfusion.A large number of transfusion patients have a higher incidence of hypocalcemia,and the calcium level needs to be closely monitored during the blood transfusion.
