摘要
目的探讨不同时期甘露醇联合甲强龙对重症手足口病患儿心肌酶和细胞因子的影响。方法选取医院2017年1月-2018年1月期间收治的60例重症手足口病患儿,按照随机数字表的方法分为观察组(n=30)和对照组(n=30),在常规治疗基础上,观察组和对照组分别在患儿入院8 h内与> 8~12 h给予以甘露醇联合甲强龙进行治疗。分别对两组患儿心肌酶水平、细胞因子水平和治疗效果进行比较。结果与对照组患儿相比,观察组患儿在心肌酶水平上的表现具有明显优势,差异有统计学意义(P <0. 05);经比较,两组患儿在术后细胞因子的浓度均有改善,观察组患儿的改善水平较为明显,差异具有统计学意义(P <0. 05);观察组患儿的总有效率达93. 33%,相较于对照组患儿的80. 00%有明显差异,具有统计学意义(P <0. 05)。结论在8 h内应用甘露醇联合甲强龙进行治疗能够有效改善患儿的心肌酶和细胞因子水平,减少肺水肿发生的情况,对于控制患儿病情有着显著效果,值得被推广与应用。
Objective To investigate the effects of mannitol combined with methylprednisolone on myocardial enzymes and cytokines in children with severe hand,foot and mouth disease at different stages. Methods Sixty children with severe hand,foot and mouth disease admitted to our hospital from January 2017 to January 2018 were divided into observation group( n = 30) and control group( n = 30) with a random number table. The two groups were treated symptomatically. The observation group and control group were treated with mannitol and methylprednisolone within 8 hours and > 8-12 hours,respectively. The levels of myocardial enzymes,cytokines and therapeutic effects were compared between the two groups. Results Compared with the control group,the performance of myocardial enzymes in the observation group had obvious advantages and significant difference( P < 0. 05);After comparison,the concentration of cytokines in the two groups were both improved,and the improvement level of the observation group was more significant,statistically( P < 0. 05);The cure efficiency of the observation group was 93. 33%,compared with the control group. 80. 00% had significant difference( P < 0. 05).Conclusion Mannitol combined with methylprednisolone within 8 hours can effectively improve the level of myocardial enzymes and cytokines and reduce the occurrence of pulmonary edema in children. It is apparently effective in controlling the condition of children,which is worthy of promotion and application.
