107例大剂量盐酸羟考酮缓释片治疗中重度癌痛的有效性和安全性研究

A study of the efficacy and safety of large-dose oxycodone hydrochloride prolonged-release tablets in treating 107 cases of patients with moderate and severe cancer pain

目的探讨大剂量盐酸羟考酮缓释片(日剂量超过160 mg)治疗中重度癌痛患者的有效性和安全性。方法对2018年4月至2018年10月符合入组标准的107例患者进行回顾性研究,分析患者在达到大剂量前1天、大剂量治疗当日等5个时间点的疼痛评分、患者药物剂量和不良反应的发生情况。结果大剂量盐酸羟考酮完全缓解率(无痛患者比例)随治疗时间逐渐提高,治疗1周可达到6.54%。有效缓解率(疼痛评分≤3分患者比例)在治疗当日达到近50%,治疗1周可达到90%。患者治疗1周的盐酸羟考酮平均日剂量为228.88 mg。与大剂量盐酸羟考酮治疗前相比,大剂量盐酸羟考酮治疗后不良事件发生率的差异无统计学意义(P>0.05),治疗当日不良事件发生率最高为10.28%,随治疗时间延长而逐渐减低。结论大剂量盐酸羟考酮可有效缓解中重度疼痛,患者耐受性良好,值得在中重度癌痛临床治疗上进行推广。

Objective To investigate the efficacy and safety of large-dose oxycodone hydrochloride prolonged-release tablets(daily dose exceeding 160 mg)in patients with moderate and severe cancer pain.Methods A retrospective study was conducted on 107 cases of patients conforming to the entry criteria from April 2018 to October 2018.The pain score,drug dose and adverse reactions of the patients were analyzed at five time points including one day before reaching the large dose and the day of large dose treatment.Results The complete remission rate of large dose oxycodone hydrochloride(the proportion of painless patients)gradually increased with the passage of the treatment time,and reached 6.54%after one week.The effective remission rate(the proportion of patients with pain score no more than 3 points)reached nearly 50%on the day of treatment and 90%at one week of treatment.The average daily dose of oxycodone hydrochloride for one week of treatment was 228.88 mg.There was no difference in adverse event rate between time points before and after treatment with large dose oxycodone hydrochloride.The highest adverse event rate on the day of treatment was 10.28%,which gradually decreased with the treatment.Conclusion Large dose oxycodone hydrochloride can effectively relieve various types of moderate and severe pains with good patient tolerance,worth popularizing in the clinical treatment of moderate and severe cancer pain.
