综合护理联合怀旧疗法对老年2型糖尿病伴抑郁症患者血糖水平及抑郁情绪的影响

Effects of comprehensive nursing combined with reminiscence therapy on blood glucose and depressive level of elderly patients with type 2 diabetes and depression

目的探讨综合护理联合怀旧疗法对老年2型糖尿病伴抑郁症患者血糖水平及抑郁情绪的影响。方法选择2014年6月-2017年6月我中心收治的老年2型糖尿病伴抑郁症患者98例,按随机数字表法将其分为对照组与观察组各49例。对照组予以常规护理,观察组在常规护理基础上予以综合护理联合怀旧疗法进行干预,2组均干预6个月。对比2组干预前后血糖[空腹血糖(FBG)、餐后2h血糖(2hPG)]水平,抑郁自评量表(self-rating depression scale,SDS)、简易生活质量评估表(the MOS item short from health survey,SF-36)得分以及治疗依从性。结果干预后,观察组FBG、2hPG水平、SDS评分均低于对照组(t=10.815,P<0.001;t=3.815,P<0.001;t=4.579,P<0.001),SF-36评分高于对照组(t=4.516,P<0.001);依从性好于对照组(Z=2.538,P=0.011)。结论综合护理联合怀旧疗法可显著降低老年2型糖尿病伴抑郁症患者血糖水平,缓解患者抑郁情绪,提高其治疗依从性及生活质量。

Objective To investigate the effect of comprehensive nursing combined with reminiscence therapy on the blood glucose level and depressive mood of elderly patients with type 2 diabetes and depression.Methods A total of 98 patients with type 2 diabetes and depression treated in our center between June 2014 and June 2017 were randomly divided into a control group(n=49)and an observation group(n=49).Both groups were given routine nursing,while the observation group was additionally provided with comprehensive nursing combined with reminiscence therapy for6 months.The level of blood glucose[fasting blood glucose(FBG),postprandial 2 hblood glucose(2 hPG)],self-rating depression scale(SDS),the MOS item short from health survey(SF-36)and treatment compliance before and after intervention were compared between the two groups.Results After the intervention,the levels of FBG 2 hPG and SDS score of the observation group were significantly lower than that of the control group(t=10.815,P<0.001;t=3.815,P<0.001;t=4.579,P <0.001),while the SF-36 score and compliance rate of the former group were significantly higher than those of the latter group(t=4.516,P<0.001;Z=2.538,P=0.011).Conclusion Comprehensive nursing combined with reminiscence therapy can significantly reduce the level of blood glucose,relieve the depression of the patients,and improve the compliance and life quality of elderly patients with type 2 diabetes and depression.
