居家脑卒中病人护理分级评估指标体系的构建

Construction of evaluation indicator system for nursing grading of patients with stroke at home

[目的]构建基于生理‐心理‐社会医学模式的居家脑卒中病人护理分级评估指标体系,为居家脑卒中病人的护理分级提供依据。[方法]基于文献分析、质性访谈、专家小组会议初步确定居家脑卒中病人护理分级评估指标体系;于2018年6月—9月邀请19名脑卒中护理领域相关专家进行3轮专家咨询,采用层次分析法确定各指标权重。[结果]3轮咨询专家的积极系数分别为85.00%、89.47%、100.00%;专家权威系数分别为0.878,0.879,0.879;第3轮专家意见协调系数分别为0.393,0.366,0.388;最终形成包括病情严重程度、日常生活活动能力、精神心理状态、社会支持与环境资源利用能力4个一级指标、30个二级指标和120条评价标准的居家脑卒中病人护理分级评估指标体系。[结论]构建的居家脑卒中病人护理分级评估指标体系具有较强的科学性、可靠性,可为居家脑卒中病人的护理分级提供理论依据。

Objective:To construct the nursing grading evaluation indicator system for stroke patients at home based on physiologicalpsychological-social medicine model,so as to provide evaluation basis for nursing grading of stroke patients at home.Methods:Through literature analysis,qualitative interview and expert group meeting,the preliminary draft of the nursing grading evaluation indicator system was prepared.A total of 19 experts related to stroke care were invited to complete three rounds of expert consultation from June,2018 to September,2018,and the weight of each indicator was determined by the analytic hierarchy process.Results:The positive coefficients of the three rounds of expert consultation were 85.00%,89.47%and 100.00%,respectively.The expert authority coefficients were 0.878,0.879 and 0.879,respectively.The degree of coordination of expert opinions were 0.393,0.366,0.388,respectively.The final nursing grading evaluation indicator system included four first-level indicators,which were severity of illness,ability of daily life activities,mental state,social support and environmental resource utilization,30 second-level indicators and 120 evaluation criteria.Conclusions:The nursing grading evaluation indictor system of stroke patients at home established in this study was scientific and reliable,which can provide theoretical basis for the nursing grading of patients with stroke at home.
