某三甲医院医疗机构制剂医保未收录后使用情况分析

Analysis of the use of hospital preparations after removing from medical insurance list in a third-class hospital

目的:分析北京大学第三医院(以下简称为我院)医疗机构制剂从2013年医保未收录后的使用情况,探讨价格因素对医疗机构制剂的影响,以及医疗机构制剂存在的必要性及发展方向。方法:通过医疗机构制剂信息管理系统,以2012年3月1日至2013年2月28日调整价格前1年为基数,采用Excel软件对医保未收录后我院医疗机构制剂5年使用量进行统计。结果:5年间,我院医疗机构制剂品种的配制与使用量总体呈增长趋势。中药制剂用药金额构成比逐年增加,其中颈痛平片、复方苍耳子片、创伤止痛乳膏等特色中药制剂呈现良好增长态势。价格调整医保未收录后,部分中药和西药制剂被商品药替代,用量骤减,最终停产。结论:继续保持现有医疗机构制剂的稳定生产并积极开发特色中药制剂,既可满足临床患者的用药需求,也是我院医疗机构制剂生存和发展的方向。

Objective:The hospital preparations of Peking University Third Hospital(hereinafter referred to as our hospital) were analyzed after removing from medical insurance list since 2013.The impact of price on the hospital preparations was explored,together with the necessity and the development direction of the hospital preparations.Methods:Data was collected from the pharmaceutical information system of medical institutions.The five-year usage after removing from medical insurance list were calculated by Excel software,based on the usage before the price adjustment that was from March 1,2012 to February 28,2013.Results:During these five years,both the number and the production of preparations in our hospital had an increased trend.The proportion of the amount in traditional Chinese medicine preparations increased year by year,especially in Jingtongping Pian,Fufang Cangerzi Pian and Chuangshangzhitong Rugao.Some preparations were replaced by commercial drugs after the price adjustment and being withdrawn from medical reimbursement insurance.Their usages dropped sharply and then was not in production.Conclusion:To remain the stable production of the existing medical institution preparations and actively develop the traditional Chinese medicine preparations with features could not only meet the patients’ needs,but also be the direction of the survival and development of the medical institution preparations in our hospital.
